摘 要:目的分析糖尿病患者血培养阳性患者病原体分布特点及耐药的风险因素。方法选取2012年10月-2017年10月在本院就诊的糖尿病血培养阳性患者112例作为研究对象,分析患者血培养病原体分布特点,并对患者药敏结果进行分析。结果草绿色杆菌、金黄色葡萄球菌、鲍曼不动杆菌耐药率分别为35.3%、26.9%、24.0%,金黄色葡萄球菌、草绿色杆菌、鲍曼不动杆菌对头孢曲松、头孢噻肟、氨曲南、莫西沙星具有较高的耐药率,金黄色葡萄球菌、草绿色杆菌、鲍曼不动杆菌对美罗培南的耐药率分别为8.3%、14.2%、0.0%,对万古霉素的耐药率分别为16.7%、14.3%、0.0%。住院时间、胰岛素抵抗、糖尿病病程是患者耐药菌感染的独立危险因素(P<0.05)。结论糖尿病患者具有较高的耐药细菌感染率,病程长、住院时间长是耐药感染的风险因素。Objective To analyze the distribution characteristics and drug resistance of pathogens in diabetic patients with positive blood culture.Methods 112 cases of diabetic patients with positive blood culture who were admitted to our hospital from October 2012 to October 2017 were selected as subjects.The distribution characteristics of pathogens and the drug susceptibility results were analyzed.Results The resistance rates of Bacterium viridis,Staphylococcus aureus and Acinetobacter baumannii were 35.3%,26.9% and 24.0%,respectively.They had higher resistance to cefixime,cefotaxime,aztreonam and moxifloxacin.Their resistance rate was respectively 8.3%,14.2%,and 0.0% to meropenem,and 16.7%,14.3%,and 0.0% to vancomycin.Hospitalization time,insulin resistance,and diabetic duration were independent risk factors for drug-resistant bacterial infection(P<0.05).Conclusion Diabetic patients have a higher infection rate to resistant bacteria.Long diabetic duration and long hospital stay are risk factors for drug-resistant infection.
