17511例泌尿生殖道支原体检测及药敏结果分析

摘　　要：目的分析本院2015年-2017年17 511例泌尿生殖道支原体感染及药敏情况,为临床合理使用抗菌药物提供依据。方法采用支原体培养鉴定和药敏试剂盒对送检的标本进行培养和药敏试验,并对结果进行回顾分析。结果 2015年-2017年单一解脲支原体(Uu)的检出率为47.12%,单一人型支原体(Mh)检出率为1.54%,Uu的检出率高于Mh,差异有统计学意义(P<0.01),其中女性感染率为66.06%,男性感染率为28.72%,女性感染率高于男性(P<0.01)。单一Uu和单一Mh对强力霉素、交沙霉素、美满霉素的敏感性高,敏感率均>97%。单一Mh阳性的标本和Uu+Mh复合感染的标本对大环内酯类抗菌药物耐药率明显高于单一Uu阳性标本(P<0.01)。Mh阳性的标本对喹诺酮类抗菌药物的耐药率高于单一Uu阳性的标本(P<0.01)。结论 Uu的检出率高于Mh,且女性感染率高于男性。经验用药应避免使用大环内酯类、喹诺酮类抗菌药物,并针对支原体的药敏结果合理选择用药。Objective To analyze the infection and drug sensitivity of 17 511 cases of urogenital Mycoplasma in our hospital from 2015 to 2017,and provide basis for clinical rational use of antibiotics.Methods The collected samples were conducted for culture and drug susceptibility test by Mycoplasma culture identification and drug sensitivity kit,and the results were retrospectively analyzed.Results The detection rate of single Ureaplasma urealyticum(Uu) was 47.12% while the detection rate of single human Mycoplasma(Mh) was 1.54%.The detection rate of Uu was significantly higher than that of Mh,with the difference statistically significant(P<0.01),in which the female infection rate was 66.06%,male infection rate was 28.72%,which were significantly higher in females than that in males(P<0.01).Single Uu and single Mh had high sensitivity to doxycycline,josamycin,and minocycline,and the sensitivity rate of which was >97%.The resistance rate of macrolide antibiotics to single Mh-positive specimens and Uu+Mh-complexed specimens was significantly higher than that of single Uu-positive specimens(P<0.01).The resistance rate of Mh-positive specimens to quinolone antibiotics was higher than that of single Uu-positive specimens(P<0.01).Conclusion The detection rate of Uu is higher than Mh,and the female infection rate is higher than that of males.The use of macrolides,quinolone antibiotics should be avoided,and rational choice of medication should be based on the drug susceptibility results of Mycoplasma.
