摘　　要：本文通过全程药学监护1例长期口服华法林患者的围手术期桥接抗凝的治疗,并对其桥接抗凝治疗方案进行分析。根据患者个体情况,制定合理有效的围手术期桥接抗凝管理方案,最大限度的降低了患者围手术期的出血与血栓栓塞风险。结果表明,临床药师通过有效的药学监护服务,在优化患者的围手术期桥接抗凝治疗方案中可以发挥重要作用。In the paper,a perioperative bridging anticoagulation therapy was performed in a long-term oral warfarin treated patient with full-scale pharmaceutical monitoring,and the bridging anticoagulant therapy was analyzed. According to the individual conditions of the patient,a reasonable and effective perioperative bridging anticoagulation management program was developed to minimize the risk of perioperative bleeding and thromboembolism. The results showed that clinical pharmacists can play an important role in optimizing perioperative bridging anticoagulant therapy for patients through effective pharmaceutical services.
