摘  要:目的构建新型经阴道超声检测内膜厚度及形态变化系数模型,预测体外受精-胚胎移植临床结局。方法回顾分析403个IVF移植周期的妊娠结局,评估经阴超子宫内膜厚度变化系数(T-VEm)和内膜形态变化系数(MVEm)预期妊娠结局的临床价值。结果鲜胚和冻融人工周期,T-VEm≥3.5mm妊娠率和活产率升高,流产率显著下降。冻融自然周期T-VEm≥3.5mm仅妊娠率升高。鲜胚和冻融自然周期M-VEm>80%,活产率显著下降、流产率明显上升。冻融人工周期M-VEm在0.5~0.8mm之间的活产率明显增高,流产率降低。结论转化日和移植日内膜均是预测妊娠结局的独立影响因子,而转化日至移植日内膜的厚度和形态变化更直接反映转化效率,预测妊娠结局更准确。Objective To construct the new pattern of morphologic and thickness variance of endometrium using ultrasonography to predict the IVF-ET clinical outcome.Methods The study retrospectively review the 403 IVF-ET cycles and constructed a new predictor to analyze the endometrial change between days of conversion and implantation:T-VEm and M-VEm,and further validate their clinical value.Results In the fET and HRT+FET,higher T-VEm(≥3.5 mm)manifested an evident increase in clinical pregnancy rates and live birth rates,and an decrease in abortion rates.In the NC+FET,higher T-VEm(≥3.5 mm)significantly improved in clinical pregnancy rates,but no change in live birth and abortion rates.In fET and NC+FET,the live birth rates declined in M-Em≥80% group,and abortion rates increased.The M-Em(0.5-0.8 mm)group using HRT+fET had a higher live birth rate and lower abortion rate.Conclusions We invented novel mathematic formula to analyze the morphological and thickness changes of endometrium between the days of conversion and implantation,which is a better predictor to the clinical outcome.
