基于OHT健康规划工具的新生儿安全干预策略成本-效果预评估

摘　　要：目的:利用One Health Tool (OHT)健康规划软件,建立"新生儿安全项目"宁夏项目县干预模型并对干预策略进行成本-效果分析,形成项目干预前预评估证据。方法:基于基线调研获取的各指标数据,在OHT工具中建立预测模型,设定人口学特征、人群健康、覆盖率等相关参数,依次得到项目干预可预期的效果及成本预测情况。结果:软件预测实施新生儿安全项目干预后,至2020年宁夏3个项目县的新生儿死亡率将明显降低。Y县实施项目的总成本最高(444.49万元),D县最低(114.75万元),S县更具成本效果。结论:具体干预策略中袋鼠妈妈护理、新生儿眼部护理、产后出血预防、对胎膜早破新生儿使用抗生素、6个月内纯母乳喂养、延迟脐带结扎和正确护理等策略明显更加具有成本效果。Objective: One Health Tool(OHT) Health Planning Software was applied to set up the intervention model of "neonatal security project" in the project county of Ningxia and form the evidence for the pre-assessment of the project. Methods: The research employed the One Health Tool software to finish the assessment based on different indexes of literature review and frontline survey.Parameters like demographic indictors, health status of population and intervention coverage rate were essential to the predictable intervention effects and cost prediction of assessment research. Results: After the implementing the project of neonatal security intervention, the mortality of neonatal in three counties would significantly decrease in Ningxia untial 2020. The total cost of the project ranged from 1 147.5 thousand yuan(County D) to 4 444.9 thousand yuan(County Y), and the project in County S would be the most cost-effective. Conclusion: Kangaroo mother care, eye-care of new-born, pure breastfeeding within 6 months, delayed umbilical cord ligation and correct nursing had obvious cost-effectiveness.
