摘　　要：目的:联合应用磁敏感加权成像(SWI)与三维动脉自旋标记成像(3D ASL)技术,探讨其对轻型颅脑损伤(mTBl)患者的临床应用价值。方法:收集45例符合急性轻型颅脑损伤的患者与25例正常体检者纳入本项研究,两组均行常规MRI及SWI与3DASL检査,并在检査3个月后进行脑震荡综合征问卷(RPQ)评分,观察影像表现及其与脑震荡综合征(PCS)发生率的关系。结果:本组患者PCS阳性15例,SWI检出微出血灶13例,其中8例⑹.53%)PCS阳性;出现局部脑血流灌注减低区21例,其中12例(57.14%)PCS阳性。存在脑微出血及(或)局部灌注异常组出现PCS阳性概率明显高于无明显异常组(P<0.01)。结论:联合SWI、3D ASL可检出轻型颅脑损伤患者出现的脑微出血及脑血流灌注异常,出现这些异常的患者较易迁延成脑震荡综合征,及时检出与处理对于改善患者预后具有重要意义。Purpose: To assess the value of SWI in combination with 3D ASL in the evaluation of mild traumatic brain injury (mTBI). Methods: Forty-five mTBI patients and 25 healthy volunteers were respectively selected as case group and control group respectively, All the subjects were performed with conventional, SWI and 3D ASL sequences, and their post-concussion syndrome (PCS) RPQ scores of them were also recorded. Results: Twentyone cases with regional hypoperfusion, 13 cases with microbleeds were found in the 45 mTBI patients and 15 mTBI patients with RPQ score >14 points were PCS positive. Of the 21 patients with local rCBF hypoperfusion, 12 (57.14%) were with PCS positive. Of the 13 patients with microbleeds, 8 (61.53%) were with PCS positive. The probability of occurrence of PCS in the patients with cerebral microbleeds and/or local perfusion abnormal group was significantly higher than those without any obvious abnormality (P<0.01). Conclusion: SWI combined with 3D ASL imaging can be used to quantitatively detect the microbleeds and the rCBF abnormality in patients with mTBI. It is of great significance to detect and correct the abnormal brain microcirculation and improve the prognosis of the patients.
