摘　　要：目的:探讨慢性阻塞性肺病(COPD)患者与其冠状动脉钙化积分(CACS)之间的关系。方法:回顾分析100例COPD患者临床资料、功能检査、胸部CT资料及冠状动脉钙化积分情况,先对其CACS与相应临床资料、功能检查及%LAA-950 (CT值低于-950HU所占百分比,反映肺气肿程度)进行二元Logistic相关与回归检测;再对冠脉钙化分析组(CACS〉100、CACS W 100)与冠脉无钙化对照组(CACS=0)各分析指标采用组间均数比较独立样本t检验或t'检验。结果:①COPD患者CACS与其年龄、%LAA-950、FEV1和合并并发症有明显相关性;②冠脉钙化分析组及无钙化对照组组间均数比较结果显示,患者年龄、FEV1、FEV1/FEV1-pre.、FEV1/FVC、FVC、%LAA-950组间比较差异有统计学意义;③冠脉钙化分析组内CACS>100组中年龄、%LAA-950值较大,FEV1值较低。结论:COPD与冠心病关系密切,其患病风险预测因素包括年龄、%LAA-950、FEV1及并发症情况。Purpose: To investigate the association between patients with chronic obstructive pulmonary disease (COPD) and coronary artery calcification score (CACS). Methods: The clinical data, function test, chest CT and CACS of 100 patients with mild and moderate COPD were analyzed retrospectively. The binary Logistic regression analysis was performed for CACS and corresponding clinical data, functional examination and %LAA-950 (the percentage of CT value below -950Hu, reflecting the degree of emphysema) firstly, then independent sample t-test or t'-test was used to compare the mean value of analysis indices between coronary artery calcification groups (CACS>100 and CACS W 100) and coronary artery calcification free group (CACS=0). Results:1.The CACS of patients with COPD significantly correlated with their age,%LAA-950, FEV1 and complications.2.The comparison between the coronary calcification group and the non-calcification control group showed that age, FEV1, FEV1/FEV1-pre, FEV1/FVC, FVC and %LAA-950 were with significant differences.3.In coronary artery calcification group, age and %LAA-950 value of the group with CACS>100 were larger and FEV1 value was lower. Conclusion: COPD is closely related to coronary heart disease, and its predictors of the risk of disease include age,%LAA-950, FEV1 and complications.
