摘 要:目的:探讨3.0T MRI跌软骨损伤与正常软骨T2值差异,并分析各分期跌软骨T2值差异。方法:对801例膝关节MR检査者在常规膝关节检查序列基础上,采用饌骨矢状位T2-mapping成像。利用Rose分期对濮软骨损伤进行MRI分期,利用Sigvia后处理软件测量跌软骨显示最清晰的连续三个层面T2值。结果:MRI男女正常组的T2值差异无统计学意义(P>0.05),離软骨病损发生时,病例组T2值明显高于正常组(PV0.001),Ⅰ~Ⅳ期験软骨病损的T2值也明显高于正常组(P<0.001),Ⅰ~Ⅳ期饌软骨病损的T2值之间差异有统计学意义(P< 0.001),且T2值随病变分期升高而增加。各分期中,男女性病例组的T2值均高于正常组。男女性别组各期T2值差异不显著(P>0.05)。男女性饌软骨病损随分期升高而增加,仅男性Ⅲ期与Ⅳ期的差异不显著(P>0.05)。利用T2-mapping色阶图能直观地显示跌软骨病损的程度,随分期升高,饌软骨蓝色区域减少,绿色及红色区域随之增加。结论:MRI正常及各期病例组離软骨T2值与性别无相关性,擴软骨退变时T2值随病变分期升高而明显增加。MR T2-mapping成像通过色阶图及T2值能对験软骨病损进行分析,对饌软骨病损的诊断、分期与评估具有重要的临床意义。Purpose: To investigate the difference of T2 value between injured patellar cartilage and normal cartilage on 3.0T MRI and to analyze the T2 value of patellar cartilage lesion in different stages. Methods: The sagittal T2- mapping imaging of patella was performed in 801 knees based on routine knee examination sequence. The patellar cartilage injury was staged by Rose stage, and the T2 value of patellar cartilage was measured on the clearest continuous three layers of patellar cartilage on Sigvia workstation. Results: There was no significant difference in T2 value between male and female normal group on MRI (P>0.05). In patellar cartilage lesions, the value of T2 in the patellar cartilage lesions was significantly higher than that in the normal group (P<0.001). There were significant differences in T2 value of patellar cartilage lesions among Ⅰ-Ⅳ stages (P<0.001). The T2 value increased with the stage increase of lesions. In each stage, the T2 value of male and female patients was higher than that of those in the normal group. There was no significant difference in T2 value between male and female groups in each stage (P>0.05). The T2 value of patellar cartilage lesions in males and females increased with the increase of stage, but there was no significant difference between stage Ⅲ and Ⅳ in males (P>0.05). The degree of patellar cartilage lesion can be displayed by T2 mapping. With the increase of stages, the blue area of patellar cartilage decreased, and the green and red area increased. Conclusion: There was no correlation between patellar cartilage T2 value and gender in normal and injured groups. The T2 value increased significantly with the increase of pathological stages in patellar cartilage degeneration. T2-mapping can be used to analyze the patellar cartilage lesions, which has important clinical significance in the diagnosis, staging and evaluation of patellar cartilage lesions.
