摘　　要：目的观察锁定接骨板治疗陈旧性桡骨远端骨折的临床疗效。方法自2010年1月至2016年3月用掌侧或背侧锁定接骨板治疗陈旧性桡骨远端骨折并外观畸形,取得有效回访共38例,手术年龄范围17~77岁,平均年龄51.1岁;按AO分型:A型16例,B型10例,C型12例,其中27例采取掌侧手术入路,11例采用背侧入路。结果38例均得到随访,平均随访时间为57.1个月(15.6~90.0个月)。按Cooney腕关节评分标准:优21例,良12例,可5例,优良率为86.8%。结论桡骨远端骨折畸形愈合明显影响腕关节功能活动,治疗的重点是尽量恢复桡骨远端的解剖角度和关节面的平整,术后积极的康复锻炼可促进腕关节的功能恢复。Objective To observe the clinical efficacy of locking plate in the treatment of old distal radius fracture. Methods From January, 2010 to March, 2016, 38 cases of old distal radius fractures were treated with volar or dorsal locking plate. They were 17-77 years old, with a mean age of 51.1. According to AO classification, there were 16 cases of type A, 10 type B, and 12 type C. 27 cases were operated by volar approach, and the other 11 cases by dorsal approach. Results The mean follow-up period was 57.1 months (15.6 to 90 months). According to Cooney wrist score standard, the results were excellent in 21 cases, good in 12, and fair 5 cases, with an excellent and good rate of 86.8%. Conclusion The malunion of old distal radius fracture significantly affects the wrist joint function. The treatment should focus on restoring the anatomical angle of the distal radius and the articular surface smoothness. Active rehabilitation exercises after operation can promote the functional recovery of wrist joint.
