米非司酮对子宫肌瘤及其肌层组织中生化指标水平的影响

摘　　要：目的探讨子宫肌瘤术前应用米非司酮对肌瘤及其肌层组织生化指标水平的影响。方法回顾性分析124例子宫肌瘤患者的临床资料,86例术前应用米非司酮者纳入观察组,1日1次,自月经第1天开始服用3个月;38例其他患者纳入对照组。结果初诊3个月后,观察组子宫螺旋动脉搏动指数(PI)、收缩期峰值流速/舒张期峰值流速(S/D)水平及子宫体积均较初诊时显著下降,且明显低于对照组(P<0.05);观察组子宫肌瘤组织雌激素受体(ER)、孕激素受体(PR)水平均明显低于对照组,肿瘤坏死因子-α(TNF-α)水平明显高于对照组(P<0.05);观察组子宫正常肌层组织ER,PR,TNF-α水平均明显低于对照组(P<0.05);观察组手术时间、术后住院时间均短于对照组,术中出血量、术后输血例数均明显少于对照组(P<0.05)。结论米非司酮可有效减少子宫肌瘤及正常肌层组织ER和PR水平,对术前阻碍子宫肌瘤进展有利,能为提升其手术安全性创造条件。Objective To investigate the effect of mifepristone on the biochemical indexes of uterine fibroid and its myometrial tissue before operation.Methods The clinical data of 124 patients with uterine fibroids were retrospectively analyzed,86 patients who received mifepristone before operation were included in the observation group,once a day for 3 months from the first day of menstruation,and 38 other patients were included in the control group.Results Three months after first visit,the levels of pulsatility index(PI),systolic peak velocity/diastolic peak velocity(S/D)of the uterine spiral artery and the uterine volume in the observation group were significantly lower than those at first visit,which were also significantly lower than those in the control group(P<0.05).The levels of estrogen receptor(ER)and progestin receptor(PR)of the uterine fibroid tissue in the observation group were significantly lower than those in the control group,while the level of tumor necrosis factor-α(TNF-α)in the observation group was significantly higher than that in the control group(P<0.05).The levels of ER,PR and TNF-αof the normal myometrial tissue in the observation group were significantly lower than those in the control group(P<0.05).The operative time and hospital stay in the observation group were significantly shorter than those in the control group,the intraoperative blood loss and the number of postoperative blood transfusions in the observation group were significantly less/fewer than those in the control group(P<0.05).Conclusion Mifepristone can effectively reduce the levels of ER and PR of fibroid and normal myometrial tissue in patients with uterine fibroids,which is beneficial to impeding the advancement of uterine fibroid before operation,and can create conditions for improving the safety of operation.
