摘 要:目的 探讨血浆人胎盘催乳素(hPL)mRNA水平联合三维超声监测对瘢痕妊娠孕妇前置胎盘合并胎盘植入的诊断价值。方法 对2017年1—12月收治的瘢痕子宫产妇300例临床资料进行回顾性分析,根据检查结果发现前置胎盘有66例,伴有胎盘植入16例。所有患者均给予三维超声监测联合血浆hPLmRNA水平检测,比较不同检测方式的诊断符合率。结果 合并胎盘植入组孕18~24周、30~32周血浆hPLmRNA水平高于无胎盘植入组(P<0.05)。三维超声联合血浆hPLmRNA水平检测的灵敏度、特异度、阳性预测值、阴性预测值及诊断符合率均高于三维超声或hPLmRNA单一方法检测(P<0.05)。结论 血浆hPLmRNA水平联合三维超声监测对瘢痕妊娠孕妇前置胎盘合并胎盘植入的诊断符合率更高。Objective To investigate value of plasma human placental lactogen (hPL) messenger ribonucleic acid (mRNA) level detection combined with three-dimensional ultrasound (3D-U) monitoring in diagnosis of pregnant women with scar pregnancy complicated by placenta previa and implantation. Methods Clinical data of300 pregnant women with scarred uterus admitted during January2017 and December2017 was retrospectively analyzed. There were 66 pregnant women with placenta previa after examinations, among whom16 pregnant women had placenta implantation. All patients were given3D-U monitoring combined with plasma hPL mRNA level detection. Diagnose accordance rates by different detection methods were compared. Results Levels of plasma hPL mRNA in pregnant women complicated by placenta implantation in18-24 and30-32 gestational weeks were significantly higher than those in pregnant women without placenta implantation ( P <0.05). Values of sensitivity, specificity, positive predictive value, negative predictive value and diagnostic coincidence rate by 3D-U monitoring combined with plasma hPL mRNA level detection were significantly higher than those by3D-U monitoring or plasma hPL mRNA level detection only ( P <0.05). Conclusion Plasma hPL mRNA level detection combined with3D-U monitoring has a higher diagnose accordance rate in diagnosis of pregnant women with scar pregnancy complicated by placenta previa and implantation.
