摘　　要：目的调查永久性结肠造口患者的心理痛苦现状,并分析其影响因素.方法采用便利抽样法,选取2017年1月—2018年1月于郑州大学第二附属医院就诊的永久性肠造口患者112例,收集一般资料,采用心理痛苦温度计(DT)和心理痛苦相关调查表(PL)调查研究对象的心理痛苦状况.采用多元线性回归分析永久性结肠造口患者心理痛苦水平的影响因素.结果永久性结肠造口患者心理痛苦总得分为1~10分,平均得分为(6.03±2.20)分,问题列表各维度得分最高为交往问题(1.56±0.45)分,得分最低为宗教/信仰问题(1.04±0.26)分.不同年龄、教育背景、家庭人均月收入、是否独居、医保报销比例、有无造口并发症及睡眠状况的患者心理痛苦得分比较,差异有统计学意义(P<0.05).多元线性回归分析显示,年龄、教育程度、家庭月收入、是否合并并发症及睡眠障碍是永久性结肠造口患者心理痛苦水平的影响因素(P<0.05).结论永久性结肠造口患者心理痛苦较为严重,可能受年龄、教育程度、家庭月收入、是否合并并发症及睡眠状况多项因素影响,护理人员应该给予心理痛苦更多的关注.Objective To investigate the psychological distress among permanent colostomy patients and to analyze its influencing factors.Methods From January 2017 to January 2018,we selected 112 patients with permanent colostomy at the Second Affiliated Hospital of Zhengzhou University by convenience sampling to collect the general information,and investigate the psychological distress of patients with the Distress Thermometer(DT)and the Problem List(PL).Multiple linear regression was used to analyze the influencing factors of psychological distress among permanent colostomy patients.Results Among permanent colostomy patients,the total score of psychological distress ranged from 1 to 10 with(6.03±2.20)for the mean value.The highest score of PL was(1.56±0.45)in interaction problem and the lowest score was(1.04±0.26)in religious belief problem.There were statistical differences in the scores of psychological distress among patients with different ages,education levels,family monthly income,whether living alone or not,reimbursement rate of medical insurance,whether being with stoma complications or not and sleep status(P<0.05).Multiple linear regression analysis showed that the influencing factors of psychological distress of permanent colostomy patients included ages,education levels,family monthly income,whether being with complications or not and sleep disorders(P<0.05).Conclusions Psychological distress of permanent colostomy patients is severe which may be affected by many factors including ages,education levels,family monthly income,whether being with complications or not and sleep status.Nurses should pay more attention to psychological distress.
