摘　　要：目的:研究探讨寻常型银屑病患者血清白介素-4(IL-4)、白介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平、T细胞亚群结构与患者预后转归的关系。方法:选取2018年1月~2019年1月在本院进行治疗的寻常型银屑病患者120例为研究对象纳入观察组,另选同期在本院进行健康体检的正常受试者50例为对照组。对观察组和对照组患者的血清IL-4、IL-8、TNF-α水平、T细胞亚群结构水平进行检测比较,并对观察组患者经标准化的西医治疗,对治疗后患者不同时间的IL-4、IL-8、TNF-α水平、T细胞亚群结构进行对比。并采用Pearson检验对IL-4、IL-8、TNF-α与T细胞亚群相关性进行分析。结果:观察组IL-4、IL-8、TNF-α水平高于对照组,CD3^+、CD4^+、CD4^+/CD8^+低于对照组,CD8^+高于对照组,组间比较差异有统计学意义(P<0.05)。观察组患者经为期8周的治疗,在2周、4周、8周各项指标测定比较中,随着治疗时间的延长,IL-4、IL-8、TNF-α持续降低,CD3^+、CD4^+、CD4^+/CD8^+持续升高,CD8+持续降低,各组间比较差异有统计学意义(P<0.05)。经Pearson相关性检验,IL-4、IL-8、TNF-α与CD3^+、CD4^+、CD4^+/CD8^+呈负相关性,与CD8+呈正相关性。结论:寻常银屑病的发病与IL-4、IL-8、TNF-α水平升高及免疫功能低下有一定的关联,并可通过对IL-4、IL-8、TNF-α、T细胞亚群水平的监测以判断患者的预后效果。Objective:To study the relationship between serum levels of IL-4,IL-8,TNF-alpha,T cell subsets and prognosis in patients with psoriasis vulgaris.Methods:120 patients with psoriasis vulgaris who were treated in our hospital from January 2018 to January 2019 were selected as the study group,and 50 normal subjects who underwent health examination in our hospital during the same period were selected as the control group.The levels of serum IL-4,IL-8,TNF-alpha and T cell subsets in the observation group and the control group were detected and compared.The levels of IL-4,IL-8,TNF-alpha and T cell subsets in the observation group at different time after treatment were compared after standardized western medicine treatment.Pearson test was used to analyze the correlation between IL-4,IL-8,TNF-alpha and T cell subsets.Results:The levels of IL-4,IL-8 and TNF-alpha in the observation group were higher than those in the control group,while the levels of CD3^+,CD4^+,CD4^+/CD8^+in the observation group were lower than those in the control group,and the levels of CD8+in the observation group were higher than those in the control group.There was a significant difference between the two groups(P<0.05).In the observation group,after 8 weeks of treatment,the levels of IL-4,IL-8 and TNF-alpha continued to decrease,CD3^+,CD4^+,CD4^+/CD8^+increased and CD8+decreased with the prolongation of treatment time.There was significant difference among the groups(P<0.05).Pearson correlation test was used.IL-4,IL-8,TNF-alpha had negative correlation with CD3^+,CD4^+,CD4^+/CD8^+,and positive correlation with CD8+.Conclusion:The incidence of psoriasis vulgaris is related to the elevation of IL-4,IL-8,TNF-alpha levels and immunodeficiency.The prognosis of psoriasis vulgaris can be judged by monitoring the levels of IL-4,IL-8,TNF-alpha and T cell subsets.
