局部皮瓣在头面部外伤性皮肤软组织缺损一期修复中的应用

摘　　要：目的:探讨应用局部皮瓣一期修复头面部外伤性软组织缺损的方法与效果。方法:2017年9月-2018年8月共收治20例因外伤导致头面部皮肤软组织缺损患者,缺损面积约3.0cm×4.0cm^7.0cm×8.0cm,缺损深度为皮肤全层至骨质表面,根据创面缺损大小、形状、深度、位置和邻近组织的特点,在特定解剖区域设计合适的局部皮瓣如菱形皮瓣、易位皮瓣、旋转皮瓣、推进皮瓣等多种方法对缺损部位进行一期手术修复。结果:患者愈合率为95.00%(19/20),修复后皮肤色泽、美观度以及功能评分均明显高于修复前,差异有统计学意义(P<0.05)。20例患者满意度为95.00%(19/20),随访半年,仅1例患者由于创口感染致瘢痕增生需要二次修复。结论:对于急诊外伤所致的头面部软组织缺损,在特定解剖区域设计合适的局部皮瓣一期修复,能获得满意的面部外形和美观功能效果。Objective To investigate the method and effect of one-stage repair of traumatic soft tissue defect of head and face with local skin flap.Methods From September 2017 to August 2018,20 patients with head facial skin and soft tissue defect caused by trauma were collected,with defect area of 3.0cm×4.0cm-7.0cm×8.0cm.The defect depth was from the full layer of skin to the bone surface.According to the size,shape,depth,position and adjacent tissue characteristics of the wound defect,appropriate local flaps such as rhombic flaps,transposition flaps,rotating flaps and advancing flaps were designed in specific anatomical areas for the primary surgical repair of the defect.Results The healing rate of head and face skin soft tissue defects was 95.00%(19/20),and the skin color,aesthetics and function scores after repair were significantly higher than those before repair,with statistically significant differences(P<0.05).The satisfaction rate of the 20 patients was 95.00%(19/20).The patients were followed up for half a year,and only one patient needed secondary repair due to scar hyperplasia caused by wound infection.Conclusion For the head and facial soft tissue defect caused by emergency trauma,a suitable local skin flap is designed for onestage repair in specific anatomical areas,which can obtain satisfactory facial appearance and aesthetic function.
