摘  要:目的研究氢化可的松对脓毒症急性肺损伤大鼠肺组织糖皮质激素受体-a及核转录因子-κB(NF-κB)表达的影响。方法将大鼠随机分为生理盐水组(对照组)、脓毒症组(CLP组)、治疗组(HC组),每组各15只;CLP组和治疗组大鼠采用盲肠结扎穿孔法(CLP)法制备大鼠脓毒症模型;治疗组(HC组)从舌下静脉注射氢化可的松5mg/kg,对照组则注射等渗生理盐水。测定肺组织湿/干比重(W/D),同时用免疫组织化学法分析肺组织糖皮质激素受体-a、NF-κB表达情况及检测肿瘤坏死因子α(TNF-α)、白细胞介素1β(IL-1β)水平。结果氧合指数水平:脓毒症组和治疗组均较对照组大鼠显著降低(P<0.01),治疗组(HC组)大鼠的氧合指数则较脓毒症组(CLP组)大鼠显著升高(P<0.05)。脓毒症组和治疗组大鼠肺组织湿/干比重(W/D)、TNF-α、IL-1β、NF-κB的表达水平均较对照组大鼠显著升高(P<0.01);与脓毒症组对比,治疗组肺组织糖皮质激素受体-a的表达明显增多(P<0.05),TNF-α、IL-1β水平和肺组织NF-κB的表达均明显降低(P<0.05)。结论氢化可的松对脓毒症急性肺损伤具有一定的保护作用,其机制可能与抑制NF-κB表达有关。Objective To evaluate the effects of Hydrocortisone on Expression of glucocorticoid receptor alpha(GR-α) and NF-κB in Lung tissue of septic rats in rats with sepsis-induced acute lung injury (ALI). Methods forty-five rats were randomly divided into the control group, septic group and treatment group, with 15 rats in each group. Rats in the septic group and treatment group were prepared by cecal ligation and puncture (CLP). Rats in the treatment group were injected with 5 mg / kg hydrocortisone after operation, while the other groups were injected with isosmotic NaCl solution. The expression levels of TNF-α, IL-1β and NF-κB, GR-αin the lung tissue homogenate of rats in three groups were detected by enzymelinked immunosorbent assay. Results The oxygenation index of rats in the septic group and treatment group was significantly lower than that in the control group (both P<0.01), while the oxygenation index in the treatment group was significantly higher than that in the septic group (P<0.05). The TNF-α, IL-1β, NF-κB of rats in the septic group and treatment group was significantly higher than that in the control group (both P<0.01), while them in the treatment group was significantly lower than that in the septic group (P<0.05). Compared with septic group,the expression of GR-a was increased in treatment group. Conclusion Hydrocortisone can reduce lung tissue inflammatory reaction and and improve anoxic state in rats with acute lung injury caused by sepsis. It has a protective effect on lung tissue, and its mechanism may be related to the intervention of NF-κB expression.
