老年综合评估护理干预在老年糖尿病患者中的应用效果评价

摘　　要：目的:探讨基于老年综合评估的护理干预对老年糖尿病患者血糖控制、自护能力及预后的影响。方法:采用随机数字表法将60例糖尿病患者分为对照组和观察组,每组30例。对照组给予常规护理干预,观察组在常规护理的基础上给予基于老年综合评估的护理干预,即建立老年综合评估(Comprehensive geriatric assessment,CGA)小组后,对患者进行全面医疗评估、躯体功能、认知和情感、社会和环境4方面的综合评估后,针对问题改善常规护理流程。干预3个月后,比较两组患者血糖控制、自我护理能力、健康行为及生活质量。结果:观察组空腹血糖、餐后2h血糖及糖化血红蛋白数均显著优于对照组(P均<0.05);观察组患者自我护理能力、健康行为得分及生存质量均显著优于对照组(P均<0.05)。结论:基于老年综合评估的护理干预在老年糖尿病患者中效果显著,有效控制血糖,提高自我护理能力,改善健康行为及生活质量,为护理工作提供了可行的思路。Objective:To explore the analysis for the effect of path nursing intervention based on CGA on the glucose control,self-care ability and prognosis for elderly patients with diabetes.Methods:Randomized the patients into control and observation groups,30 in each group.The control group was given conventional nursing interventions,and the observation group was given a comprehensive nursing intervention based on the elderly based on routine care.After the establishment of the CGA team,the patients were subjected to comprehensive medical evaluation,physical function,cognitive and emotional,social and environmental aspects.After the comprehensive assessment,the routine nursing process is improved according to the problem.The main intervention method? After 3 months of intervention,blood glucose control,self-care ability,health behavior and quality of life were compared between the two groups of patients.Results:After 3 months intervention,the Fasting blood glucose,postprandial 2 h glucose and glycosylated hemoglobin of observation group were significantly better than that of control group ( P <0.05).The self-care ability,health behavior score and quality of life of observation group were significantly better than that of control group ( P <0.05).Conclusions:The effect of path nursing intervention based on CGA on the glucose control and self-care ability for elderly patients with diabetes is effective.It can effective control of blood,improve self-care ability,improve health behaviors and the quality of life,provides a feasible way of thinking for nursing work.
