重症监护护理评分系统在ICU患者分层护理中的应用

摘　　要：目的探讨重症监护护理评分系统与分层护理相结合对ICU患者的护理效果及对护士的影响。方法选取2017年1~9月在我院ICU病房住院治疗的80例患者作为对照组,2017年10月~2018年6月在ICU治疗的患者80例作为观察组,所有患者均由20例护士进行护理,对照组患者给予常规护理,观察组患者据重症监护护理评分系统对患者实施分层护理,比较两组患者护理第3、5天的护理结局以及护理质量;并采用MBI量表对实施前后护理人员的职业倦怠感进行比较。结果护理第3、5天,观察组患者生理功能、健康知识和行为、功能健康及心理社会健康各领域评分均明显高于对照组(P<0.05);观察组患者的健康教育、护理文书质量、危重患者护理、基础护理质量、护理技术操作每各维度评分均明显高于对照组(P<0.05);分层护理实施后,护理人员的情绪疲倦感、工作冷漠感、无成就感各维度评分及总分均明显高于对照组(P<0.05)。结论ICU患者采用重症监护护理评分系统与分层护理相结合的护理方法有效的提升了护理质量,并对患者的行为、状态和感知方面产生了正性结局,并很好的改善了护理人员的职业倦怠感,对于患者及护理人员都具有积极作用。Objective To investigate the nursing effect of intensive care nursing scoring system combined with stratified nursing on ICU patients and its influence on nurses. Methods 80 patients in ICU wards from January to September 2017 were selected as control group,80 cases in ICU wards from October 2017 to June 2018 were selected as observation group,all patients were nursed by 20 nurses.The control group was given routine nursing,the observation group was given stratified nursing according to the intensive care nursing scoring system.Nursing outcomes and nursing quality on the 3rd and 5th day of nursing were compared between the two groups,and burnout sense of nurses before and after the implementation was compared by MBI scale. Results On the 3rd and 5th day of nursing,the scores of physiological function,health knowledge and behavior,functional health and psychosocial health of patients in the observation group were significantly higher than those in the control group(P < 0.05).The scores of health education,quality of nursing documents,nursing of critical patients,quality of basic nursing and nursing technique operation in the observation group were significantly higher than those in the control group(P < 0.05).After the implementation of stratified nursing,the scores and total scores of emotional fatigue,job apathy and no sense of accomplishment of nurses were significantly lower than those of control group(P < 0.05). Conclusion Nursing quality has been effectively improved in ICU by the combination of intensive care nursing scoring system and stratified nursing,which has a positive outcome on patients’ behavior,state and perception,and has improved nurses’ job burnout,which has a positive effect on both patients and nurses.
