摘 要:目的:探讨经阴道四维子宫输卵管超声造影(4D-HyCoSy)在评估输卵管通畅性的有效性及安全性。方法:选取黄石市妇幼保健院超声科2017年1—10月接受4D-HyCoSy的患者380例,纳入在造影后1个月内进行腹腔镜下美蓝通液检查者45例。由2位医师独立判断及记录,采用卡方检验及Kappa检验分析患者一般临床特征、评估结果的差异性和一致性以及结果差异来源。结果:共纳入45例患者,有效显示90条输卵管。2位医师评估结果在不同方法中比较,差异无统计学意义(P>0.05);其一致性分析显示:4D-HyCoSy下判断输卵管通畅性Kappa=0.6382(P<0.05),腹腔镜美蓝通液检查下Kappa=0.8065(P<0.05)。对2种方法的一致性分析显示,医师1的判断Kappa=0.6336(P<0.05);医师2的判断Kappa=0.6398(P<0.05)。4D-HyCoSy检查过程中无明显疼痛、过敏反应及阴道流血。结论:4D-HyCoSy具有无创、安全、准确性高的特点,在诊断输卵管通畅性方面具有很高的价值,可作为评估不孕症患者输卵管通畅性的优先选择。Objective: To assess the availability of transvaginal Four-dimensional Hysterosalpingo-Contrast Sonography (4D HyCoSy) in evaluation of fallopian tube patency via comparison with Laparoscopic Chromopertubation. Methods: Three-hundred and eighty patients receiving 4D HyCoSy were collected from January 2017 to October 2017 and finally 45 patients with laparoscopic chromopertubation either were included.Evaluation of fallopian tube patency was performed by two clinical doctors dependently either with 4D HyCoSy or laparoscopic chromopertubation.Description analysis was applied to present the characteristics of patients and side effects.Chi-square test and Kappa analysis were applied to analyze the difference and agreement of evaluation between the doctors,respectively.Difference between the methods was analyzed by Kappa analysis. Results: Total 45 patients with 90 fallopian tubes were included.There was no significant difference between the assessment of the two doctors ( P >0.05).The agreement under 4D HyCoSy and laparoscopic chromopertubation were Kappa=0.638 2 ( P <0.05) and Kappa=0.806 5 ( P <0.05) respectively.As to the agreement of methods,Kappa=0.633 6 ( P <0.05) in doctor 1 and Kappa=0.639 8 ( P <0.05) in doctor 2.No patient was interrupted by the pain.No allergic response and apparent vaginal bleeding occurred. Conclusion: 4D HyCoSy is characterised by noninvasive,safe and highly accurate,which can be the priority for fallopian tube patency evaluation.
