摘　　要：目的对比磁共振平衡扰相梯度回波(B-FFE)及三维时间飞跃法(3D-FIESTA)在面肌痉挛患者面神经及邻近血管关系评价中的应用价值。方法选取2017年1月~2018年10月我院收治的面肌痉挛患者共60例,所有患者采用随机数字表法分为两组,每组30例,给予实验组患者磁共振B-FFE序列进行扫描诊断,对照组患者采用磁共振3D-FIESTA序列进行扫描诊断,以手术结果为金标准,对比观察两种序列扫描的诊断准确率及图像质量。结果经手术证实,两组患者均存在神经压迫,其中实验组诊断准确率(83.3%)明显高于对照组(60.0%),差异有统计学意义(P<0.05);对不同扫描所得图像进行评分,实验组明显优于对照组,差异有统计学意义(P<0.05)。结论磁共振B-FFE序列成像可清晰、准确的显示面神经及其周围血管间关系,可应用于面肌痉挛患者术前检查,指导手术方案选择,值得于临床中进一步推广使用。Objective To compare application value of contrast magnetic resonance balance fast field echo (B-FFE) sequence and 3D fast imaging employing steady state acquisition (3D-FIESTA ) sequence scan in the evaluation of the relationship between facial nerve and adjacent vessels in patients with facial spasm. Methods 60 patients with facial spasm who were admitted and treated in Mudanjiang Second People’s Hospital from January 2017 to October 2018 were selected. They were divided into two groups according to random number table method, with 30 cases in each group. Patients in the experimental group were given scan diagnosis by magnetic resonance B-FFE sequence while patients in the control group were given scan diagnosis by magnetic resonance 3D- FIESTA sequence. The diagnostic accuracy and image quality of the two sequences were compared and observed with the operation results as the gold standard. Results Surgery confirmed that there were nerve compressions in both groups, and the diagnostic accuracy of the experimental group (83.3%) was significantly higher than that of the control group (60.0%). The difference was statistically significant (P < 0.05). The images obtained from different scans were scored, and the score of experimental group was significantly better than that of the control group. The difference was significant (P < 0.05). Conclusion Magnetic resonance B-FFE sequence imaging can clearly and accurately display the relationship between facial nerve and adjacent vessels, which can be applied to preoperative examination of patients with facial spasm, guide the selection of surgical program and is worthy of further promotion and application in clinical practice.
