摘 要:目的研究2016-2017年盐城市手足口病病原CVA9型肠道病毒VP1基因分子进化特征。方法对2016-2017年盐城市疑似手足口病例标本进行病原体分子分型分析;以RT-PCR法扩增CVA9型肠道病毒VP1基因全长编码区并测序,采用生物信息学软件进行核苷酸及氨基酸序列比对及基因种系进化特征分析。结果2016-2017年盐城市共检测1 002例疑似病例标本,肠道病毒核酸阳性率38.32%,主要型别为EV71型112例(占29.17%),CVA16型94例(占24.48%),其他肠道病毒共178例(占46.35%)。检出2株CVA9型,其核苷酸、氨基酸同源性分别为94.7%、99.3%,与原型株Griggs(D00627)核苷酸、氨基酸同源性分别为80.4%~81.3%、92.7%;与引起其他症状各代表株(包括无症状健康儿童携带者)的核苷酸、氨基酸同源性为81.1%~97.2%、94.0%~99.3%。遗传进化树分析显示2毒株在C4分支中构成2条明显的传播链:CVA9/JS-yancheng/122/2016位于单独的小分支,CVA9/JS-yancheng/419/2016与2016年无症状健康儿童携带者四川株(LC361283)聚集成簇构成另一传播链。共检测31例重症病例,13例肠道病毒通用核酸阳性,阳性率41.93%,其中EV71型11例、CVA型2例。结论2016-2017年盐城市引起手足口病的CVA9病毒属于散在流行,非手足口病流行的主要基因型别,与国内部分省市引起手足口病的毒株共进化共循环,与引起其他症状类型的CVA9毒株存在差异。Objective To analyze the genetic evolutional characteristics of the VP1gene of coxsackievirus A9(CVA9)for Hand foot and mouth disease(HFMD)in Yancheng city from 2016to 2017.Methods The genotype of suspected HFMD samples collected in Yancheng city from 2016to 2017were identified by molecular subtyping analysis.The sequences of VP1complete coding region of CVA9strains were amplified by RT-PCR and the products were subjected to sequencing analysis.The comparison of nucleotide or amino acid sequences and phylogenetic analysis were performed by bioinformatics software.Results A total of 1 002suspected HFMD cases were analyzed in Yancheng city from 2016to 2017.The positive rate of enterovirus nucleic acid was 38.32%.The main genotypes were EV71subtype(112cases,accounted for 29.17%)and CVA16subtype(94cases,accounted for 24.48%),178cases(46.35%)were other enteroviruses.Totally 2strains of CVA 9subtype enterovirus were identified,the homology of nucleotide and amino acid sequences was 94.7%and 99.3%,respectively.Compared with prototype strain Griggs(D00627),the homology of nucleotide and amino acid was 80.4%-81.3%and 92.7%,respectively.Compared with representative strains causing other symptoms,including asymptomatic healthy children carriers,the homology of nucleotide and amino acid was 81.1%-97.2%and 94.0%-99.3%,respectively.Phylogenetic analysis showed that 2CVA9strains consisted 2apparent transmission chains in the C4branch:the CVA9/JS-yancheng/122/2016strain was in a separate branch;the CVA9/JS-yancheng/419/2016strain clustered with the Sichuan strain(LC361283)of asymptomatic healthy children carriers to form another transmission chain.A total of 31severe cases were detected,13were positive for enterovirus nucleic acid,the positive rate was 41.93%,including 11cases of EV71subtype and 2 cases of CVA subtype.Conclusion The CVA9enterovirus causing HFMD in Yancheng city from 2016to 2017was not dominant epidemic strain with scattered prevalence.They co-circulate with the HFMD strains in some provinces and cities in China,
