2014-2016年中山市丙肝病例重复报告及影响因素分析

摘　　要：目的分析2014-2016年中山市丙肝疫情的重复报告情况及其影响因素,指导今后的查重工作。方法对2014-2016年度报告的现住址为中山的丙肝传染病病例进行病例对照分析。结果 2014-2016年度报告的中山市丙肝病例重复报告率为31.81%(1 082/3 401);其中青少年的重复报告率为9.23%,显著低于青壮年的32.38%和老年人的31.05%(χ2=15.81,P<0.01);2016年的重复报告率为28.26%,显著低于2015年的33.46%和2014年的34.33%(χ2=11.76,P<0.01);和未分型丙肝病例相比,急性病例重复报告的风险要低(OR=0.43,95%CI:0.29-0.65);本地医疗机构比外地的丙肝重复报告风险低(OR=0.43,95%CI:0.29-0.65);传染病专科医院比综合医院的重复报告风险大(OR=1.93,95%CI:1.62-2.31)。结论青壮年及老年人慢性丙肝的重复报告情况较为严重,但整体情况已逐年好转。外地和传染病专科医院报告的丙肝病例需要传染病管理人员查重时的格外关注。Objective To reduce the repeated reports of hepatitis c. Methods A case-control analysis has been used for cases of hepatitis c which present address of the annual report of 2014-2016 in zhongshan city. Results The repeat reporting rate of hepatitis C in Zhongshan city during 2014-2016 was 31.81 %(1 082/3 401),and the rate was 9.23 % in Teenagers,32.387 % in young adults,31.054 % in old people. The rate of repeat reporting in 2016 was significantly lower than the last two years(χ~2=11.76,P<0.01). Compared with undivided hepatitis c cases,the risk of repeated reports of acute cases was lower(OR=0.43,95 % CI:0.29-0.65).The repeated reports in local medical institutions were less than those in the non-local(OR=0.43,95 % CI:0.29-0.65). The risk of repeated reports of the hospital for infectious diseases was greater than that of comprehensive hospitals(OR=1.93,95 % CI:1.62-2.31). Conclusions Repeated reports of chronic hepatitis c in young adults and older adults are more serious. But the condition has improved year by year. We should pay more attention on these cases which reported by the non-local medical institutions and by the hospital for infectious diseases.
