高频彩色多普勒超声诊断女性乳腺增生症的临床价值探讨

摘　　要：目的探讨高频彩色多普勒超声在女性乳腺增生症中的诊断价值。方法回顾性分析经高频彩色多普勒超声检查乳腺的230例女性患者的乳腺超声声像图资料。结果230例患者中,高频彩色多普勒超声检出乳腺增生症137例,占59.6%,其中小叶增生型70例(51.1%),囊性增生型25例(18.2%),纤维腺瘤样增生型23例(16.8%),导管增生型7例(5.1%),混合性病变增生型12例(8.8%)。本组137例中14例有彩色血流信号显示,占10.2%,最大流速(10.87±5.62)cm/s,最小流速(3.07±2.14) cm/s,阻力指数(RI)(0.54±0.17)。血流信号主要在肿块周边部位及部分囊壁显示,呈短棒状、细条状或闪烁点状。结论高频彩色多普勒超声应用于乳腺增生症各型诊断,诊断符合率较高,且经济、方便、无创、重复性强,具有较高的临床价值。Objective To discuss the diagnostic value of high frequency color Doppler ultrasonography in diagnosis of female mammary gland hyperplasia. Methods The ultrasonographic data of 230 female patients with breast examined by high frequency color Doppler ultrasonography were retrospectively analyzed. Results137 cases(59.6%) of mammary gland hyperplasia were detected by high frequency color Doppler ultrasonography in 230 cases, including 70 cases(51.1%) of lobular hyperplasia, 25 cases(18.2%) of cystic hyperplasia, 23 cases(16.8%) of fibroadenomatous hyperplasia, 7 cases(5.1%) of ductal hyperplasia and 12 cases(8.8%) of mixed lesion hyperplasia. Among 137 cases, 14 cases had color flow signals, accounting for 10.2%, with maximum flow velocity as(10.87±5.62) cm/s, minimum flow velocity as(3.07±2.14) cm/s, and resistance index(RI) as(0.54±0.17).The blood flow signals were mainly displayed in the periphery of the mass and part of the cystic wall, showing short rods, thin strips or scintillating dots. Conclusion The diagnostic coincidence rate of high frequency color Doppler ultrasonography is high in the diagnosis of various types of mammary gland hyperplasia, and it is economical,convenient, non-invasive and repeatable, and contains high clinical value.
