椎间孔镜下经侧后方与经椎板间隙入路手术治疗腰椎管狭窄症的效果比较

摘　　要：目的比较椎间孔镜下经侧后方入路手术与经椎板间隙入路手术治疗腰椎管狭窄症患者的临床效果。方法回顾性分析2015年5月至2017年10月在大同市第五人民医院治疗的116例腰椎管狭窄症患者的临床资料,依据椎间孔镜下手术入路方式分为对照组(椎间孔镜下经侧后方入路手术)与观察组(椎间孔镜下经椎板间隙入路手术),每组58例。比较两组患者临床指标、疼痛与功能障碍改善情况、腰椎功能。结果两组手术时间、术中出血量与住院时间比较,差异未见统计学意义(P>0.05);两组VAS评分、Oswestry功能障碍指数、腰椎功能比较,差异未见统计学意义(P>0.05)。结论腰椎管狭窄症患者通过椎间孔镜下经侧后方入路手术与经椎板间隙入路手术治疗后,治疗效果良好,临床可依据患者病情以及实际需要选择对应入路方式。Objective To compare the clinical effects of intervertebral foramen microscopy through the lateral and posterior approach and intervertebral disc space approach on lumbar spinal stenosis. Methods The clinical data of 116 patients with lumbar spinal stenosis treated in the Fifth People’s Hospital of Datong from May 2015 to October 2017 were retrospectively analyzed. According to the surgical approaches under foraminoscopy, the patients were divided into control group (translateral posterior approach under foraminoscopy) and observation group (translaminar approach under foraminoscopy), with 58 patients in each group. The clinical indicators, improvement of pain and dysfunction, and lumbar function were compared between the two groups. Results There was no significant difference between the two groups in operation time, intraoperative blood loss or hospital stay (P>0.05). There was no significant difference between the two groups in visual analogue scale score, Oswestry disability index or lumbar function (P>0.05). Conclusions The patients with lumbar spinal stenosis have good therapeutic effect after the operation through the lateral posterior approach under the foramen endoscope and through the intervertebral space approach. The corresponding approach can be selected according to the patient’s condition and actual needs.
