三维子宫输卵管超声造影在输卵管通畅性评估中的应用效果

摘　　要：目的探讨输卵管通畅性评估中三维子宫输卵管超声造影的应用效果。方法选择2017年2月至2018年9月新乡医学院第一附属医院收治的女性不孕症患者100例,均行三维子宫输卵管超声造影检查,并同时将腹腔镜通染液检查结果作为参考,分析三维子宫输卵管超声造影诊断结果及对输卵管通畅性的诊断价值。结果三维子宫输卵管超声造影诊断输卵管通畅的符合率为85.71%,通而不畅符合率为84.85%,阻塞符合率为95.65%,且Kappa=0.841,差异有统计学意义(P<0.05);通而不畅和阻塞情况为不畅通,三维子宫输卵管超声造影诊断输卵管通畅的敏感性为85.71%,阳性预测值为94.74%,准确性为96.00%,特异性为98.73%,阴性预测值为96.30%。结论在评估输卵管的通畅性中,三维子宫输卵管超声造影检查有较高的诊断价值,可为诊断不孕症提供依据。Objective To investigate the application effect of three-dimensional contrast-enhanced ultrasound of uterus and fallopian tube in the evaluation of fallopian tube patency. Methods A total of 100 cases of infertility female patients admitted in the First Affiliated Hospital of Xinxiang Medical University from February 2017 to September 2018 were selected. All patients underwent three-dimensional hysterosalpingography, and the results of laparoscopic staining fluid examination at the same time were used as reference. The diagnostic results and the value of three-dimensional hysterosalpingography in the diagnosis of fallopian tube patency were analyzed. Results The coincidence rate of three-dimensional hysterosalpingography in diagnosis of fallopian tube patency was 85.71%, which was 84.85% in diagnosis of fallopian tube unpatency, 95.65% in diagnosing fallopian tube obstruction, and by the Kappa consistency check, Kappa = 0.841, the difference was statistically significant (P<0.05);unpatency and obstruction were considered as impediment. In the diagnosis of tubal uterine tubal patency by three-dimensional ultrasonic imaging, the sensitivity was 85.71%, positive predictive value was 94.74%, the accuracy was 96.00%, specificity was 98.73% and negative predictive value was 96.30%. Conclusions Three-dimensional contrast-enhanced ultrasonography of uterus and fallopian tube has a high diagnostic value in evaluating tupe patency, which can provide a basis for the diagnosis of infertility.
