长沙市三甲医院合理用血情况调查

摘　　要：目的通过调查长沙市合理用血情况,了解长沙市合理用血现状,促进长沙市合理用血事业发展。方法收集20161月1日—12月31期年长沙市三甲医院输血病例进行整理,按照相关规范进行合理性判断,并进行统计学分析。结果该市三甲医院红细胞、血浆及血小板合理输注率分别为52.0%(2362/4542)、52.6%(782/1487)、68.1%(1165/1710),其中非手术科室红细胞、血浆、血小板合理输注率分别为60.4%(1673/2772)、56.9%(634/1114)、67.1%(1060/1580),手术科室分别为38.9%(689/1770)、17.7%(66/373)、80.8%(105/130),差异有统计学意义(P<0.05);非手术科室中血液内科血浆合理输注率仅为20.2%(69/342),ICU和肿瘤科血小板合理输注率仅为41.9%(67/160)和27.0%(17/63);手术科室中心胸外科红细胞合理输注率仅为5.3%(6/114),心胸外科、神经外科和骨科的血浆合理输注率仅为8.6%(5/58)、4.3%(2/47)和7.3%(4/55)。结论长沙市三甲医院非手术科室红细胞、血浆合理用血情况较好,血小板合理用血情况较差,手术科室血浆非合理用血情况较严重;部分科室非合理用血情况突出。该市合理用血情况仍有巨大改进空间,加强各部门之间的合作是推进合理用血事业的关键。Objective To investigate the rational use of blood in Changsha City by investigating the rational use of blood in Changsha City and promote the development of rational blood use in Changsha City. Methods Blood transfusion cases were collected from the Grade A Hospital of Changsha City in 2016,and reasonable judgments were made according to relevant norms,and statistical analysis was carried out. Results The reasonable infusion rates of red blood cells,plasma and platelets in the top three hospitals in our city were 52.0%(236 2/454 2),52.6%(782/148 7),and 68.1%(116 5/171 0),respectively. The reasonable infusion rates of plasma and platelets were 60.4%(167 3/277 2),56.9%(634/111 4),67.1%(106 0/158 0),respectively,and the surgical department was 38.9%(689/177 0),17.7%(66/373),80.8%(105/130),the difference was statistically significant(P<0.05);the reasonable infusion rate of blood medicine in non-surgical department was only 20.2%(69/342),The reasonable infusion rate of ICU and oncology platelets was only 41.9%(67/160) and 27.0%(17/63);the reasonable infusion rate of red blood cells in the thoracic surgery department of the surgical department was only 5.3%(6/114),cardiothoracic surgery,rational plasma infusion rates for neurosurgery and orthopedics were only 8.6%(5/58),4.3%(2/47),and 7.3%(4/55). Conclusion The reasonable use of red blood cells and plasma in the non-surgical department of grade A Hospital of Changsha City is better. The reasonable blood use of platelets is poor. The unreasonable blood use of the surgical department is more serious. The unreasonable blood use of some departments is outstanding. There is still room for improvement in the rational use of blood in our city. Strengthening cooperation between various departments is the key to promoting rational blood use.
