摘 要:目的为了顺利实施胫骨横向搬移的实验研究,促进Ilizarov微循环重建技术的发展,降低实验成本,自行研究设计并开发兔胫骨横向搬移系统,并探讨其用于动物实验的可行性。方法自行研究设计兔胫骨横向搬移系统,安放于4只新西兰兔左胫骨上段内侧,并实施胫骨上段横向搬移,观察其骨横搬效果。将24只新西兰兔随机分为空白对照组、模型组、贝复新组和Ilizarov组,每组6只。模型组、贝复新组和Ilizarov组建立糖尿病足溃疡模型,空白对照组建立急性创面模型。Ilizarov组进行胫骨横向搬移,贝复新组给予贝复新外用治疗,模型组及空白对照组给予生理盐水换药治疗,干预后7d进行血管化程度定量分析。造模后分别拍摄0d、7d、14d和21d的照片,计算每个时相点的创面愈合率。结果该系统成功安放于新西兰兔胫骨上段内侧,通过调节牵张器可对胫骨上段进行有效横向搬移,稳定性良好,性能优良。进行胫骨横搬后7d,取创面肉芽组织进行血管化定量分析,结果显示Ilizarov组阳性血管数量较对照组多,差异有统计学意义(P<0.01)。第14d愈合率比较,Ilizarov组、空白对照组明显快于贝复新组及模型组,差异有统计学意义(P<0.01)。结论此胫骨横向搬移系统具有制作简单、轻便易操作,价格低廉的特点,可有效实施胫骨上段横向骨搬移实验,能促进创面血管再生,加快创面的愈合,适合科研人员在胫骨上段骨横向搬移的实验研究中应用。Objective In order to successfully carry out the experimental study of lateral tibial bone transfer,promote the development of Ilizarov microcirculation reconstruction technology,and reduce the cost of the experiment,the rabbit lateral tibial bone transfer system was designed and developed by ourselves,and the feasibility of its application in animal experiments was explored. Methods The lateral bone transfer system of the rabbit tibia was researched and designed by ourselves. The system was placed on the medial side of the left upper tibia of 4 New Zealand white rabbits,lateral transfer of upper segment tibia was performed for observing the results of the bone lateral transfer. Twenty-four New Zealand rabbits were randomly divided into blank control group,model group,bFGF group and Ilizarov group,and each group consisted of 6 rabbits. Diabetic foot ulcer model was established in model group,bFGF group and Ilizarov group,and acute wound model was established in blank control group. The Ilizarov group was treated with lateral transference of the tibia,the bFGF group was treated with bFGF for external use,the model group and the blank control group were treated with saline dressing change,and the degree of vascularization was quantitatively analyzed 7 days after the intervention. After the models were developed,photos were taken on day 0,day 7,day 14 and day 21 respectively to calculate the wound healing rate at each time point. Results The system was successfully installed on the medial side of the upper segment of the tibia in New Zealand rabbits. By adjusting the distractor,the upper segment of the tibia could be effectively laterally translocated,with good stability and excellent performance. Seven days after the lateral transfer of the tibia,granulation tissues of the wound were taken for quantitative analysis of vascularization,and results showed that the number of positive blood vessels in the Ilizarov group was more than that in the control group,with statistically significant difference ( P <0.01).
