摘 要:目的探讨小儿术后不同麻醉程度下气管导管拔除对患儿心率及血压的影响。方法方便选取2017年2—9月该院收治的小儿手术治疗患者38例,随机分为两组,对照组在浅麻醉下进行气管导管拔除,研究组在深麻醉下进行气管导管拔除。对比两组患儿的心率以及血压的变化情况。结果研究组患儿术后拔管时间(12.3±2.5)min、清醒时间(11.3±1.3)min,优于对照组拔管时间(13.5±1.7)min、清醒时间(12.4±1.8)min,组间数据差异有统计学意义(t=15.923、14.609,P<0.05);研究组患儿心率拔管时(103.6±5.2)次/min、拔管后10min(93.2±3.8)次/min、拔管后20min(89.4±3.6)次/min,优于对照组拔管时(115.8±9.6)次/min、拔管后10min(101.7±7.1)次/min、拔管后20min(94.2±6.1)次/min,组间数据差异有统计学意义(t=15.216、14.524、13.964,P<0.05);研究组患儿拔管后收缩压(91.3±2.6)mmHg、舒张压(50.4±3.5)mmHg,优于对照组收缩压(93.5±8.3)mmHg、舒张压(55.6±4.5)mmHg,组间数据差异有统计学意义(t=13.347、16.942,P<0.05);研究组呛咳2例、舌后坠1例、喉痉挛2例、呼吸暂停0例,优于对照组呛咳3例、舌后坠2例、喉痉挛4例、呼吸暂停1例,组间数据差异有统计学意义(χ^2=6.246、6.732、7.428、7.258,P<0.05)。结论对小儿手术治疗患者,在深麻醉的情况下进行气管导管拔除效果较为理想,临床上应当进一步推广应用。Objective To investigate the effects of tracheal catheter extraction on heart rate and blood pressure in children with different anesthesia. Methods A total of 38 children undergoing pediatric surgery in our hospital from February to September 2017 were convenient randomly divided into two groups. The control group underwent tracheal catheter extraction under light anesthesia. The study group underwent tracheal catheter extraction under deep anesthesia. The heart rate and blood pressure of the two groups were compared. Results The extubation time (12.3±2.5) min and awake time (11.3±1.3) min were better in the study group than in the control group (13.5±1.7) min and awake time (12.4±1.8) min. There was statistical difference between the data(t=15.923, 14.609, P<0.05). In the study group, the heart rate was extubated (103.6±5.2) times/min, 10 minutes after extubation (93.2±3.8) times/min, and after extubation 20min (89.4±3.6) times/min, which was better than the control group at the time of extubation (115.8±9.6) times/min, 10 minutes after extubation (101.7±7.1) times/min, and 20 minutes after extubation (94.2±6.1) times/min, there was statistical difference between the groups(t=15.216, 14.524, 13.964, P<0.05);the systolic blood pressure (91.3±2.6) mmHg and diastolic blood pressure (50.4±3.5) mmHg in the study group were better than the control group of systolic blood pressure (93.5±8.3) mmHg, diastolic blood pressure (55.6±4.5) mmHg, there was statistical difference between the groups(t=13.347, 16.942, P<0.05);study group cough 2 cases,tongue drop 1 case, 2 cases of throat, 0 cases of apnea, better than control group 3 cases of cough, 2 cases of tongue fall, 4 cases of throat, 1 case of apnea, there was statistical difference between groups(χ^2=6.246, 6.732, 7.428, 7.258, P<0.05). Conclusion For patients with pediatric surgery, the effect of tracheal catheter extraction under ideal anesthesia is ideal, and should be further promoted and applied clinically.
