甲状腺全切除术治疗甲状腺癌的临床效果及对患者甲状旁腺激素水平的影响

摘　　要：目的观察组甲状腺癌应用甲状腺全切除术治疗的效果及对甲状旁腺激素水平的影响。方法方便选取2016年6月—2017年7月就诊于该院的98例甲状腺癌患者临床资料,按不同手术方法分两组,对照组行甲状腺次全切除术,观察组行甲状腺全切除术,比较两组手术效果及并发症等情况。结果观察组手术治疗总有效率95.92%比对照组81.63%高(χ^2=5.0178,P<0.05);观察组术后甲减、低血钙症发生率20.41%、28.57%均比对照组6.12%、10.20%高(χ^2=4.3457,5.2885,P<0.05);两组术后甲状旁腺素、血清钙离子含量均比术前下降(P<0.05),观察组甲状旁腺素(9.45±0.34)pg/ml、血清钙离子(1.25±0.18)mmol/L均比对照组含量低(t=152.5567,t=21.2372,P<0.05)。结论甲状腺全切除术治疗甲状腺癌的效果显著,但会下降患者术后甲状旁腺素水平,引起术后甲减、低血钙症等并发症。Objective To observe the effect of total thyroidectomy and the effect of parathyroid hormone on thyroid cancer. Methods Convenient select the clinical data of 98 patients with thyroid cancer who were admitted to our hospital from June 2016 to July 2017 were divided into two groups according to different surgical methods. The control group underwent subtotal thyroidectomy. The observation group underwent total thyroidectomy. The surgical results and complications of the two groups. Results The total effective rate of surgical treatment in the observation group was 95.92% higher than that in the control group 81.63%(χ^2=5.017 8,P<0.05). The incidence of hypothyroidism and hypocalcemia in the observation group was 20.41% and 28.57%, which were 6.12%, 10.20% higher than the control group (χ^2=4.345 7,χ^2=5.288 5,P<0.05);the levels of parathyroid hormone and serum calcium in the two groups were lower than those before surgery (P<0.05), and the observation group was parathyroid hormone (9.45±0.34) pg/mL and serum calcium ion (1.25±0.18) mmol/L were lower than the control group (t=152.556 7, t=21.237 2,P<0.05). Conclusion Total thyroidectomy is effective in the treatment of thyroid cancer, but it will reduce the postoperative parathyroid hormone level, causing complications such as postoperative hypothyroidism and hypocalcemia.
