医院-社区联合模式对脑卒中气管切开带管患者吸痰管理的应用研究

摘　　要：目的研究分析对脑卒中气管切开带管患者采取医院-社区联合模式吸痰管理的临床疗效及应用价值。方法选择2016年06月至2018年12月期间我院气管切开带管出院的脑卒中40例患者。将患者随机分为两组,每组各20例。研究组实施医院-社区联合模式进行吸痰护理随访,对照组实施常规出院模式行吸痰护理指导与宣教。记录并对比两组脑卒中气管切开带管患者的痰液吸净时间、住院时间、护理满意度、再次入院率及并发症评分。结果观察组痰液吸净时间、住院时间短于对照组,护理满意度高于对照组,出院后6个月内的再次入院率及并发症评分低于对照组,P<0.05。结论采取了医院-社区联合模式吸痰管理的脑卒中气管切开带管患者,吸痰时间与住院时间明显缩短,护理满意度高,且再次入院率及并发症发生率低,值得推广。Objective:To study the clinical efficacy and application value of hospital-community combined mode suction management for stroke patients with tracheotomy.Methods Forty patients with strokes who were discharged from the tracheotomy tube in our hospital from June 2016 to December 2018 were enrolled.Patients were randomized into two groups of 20 patients each.The research group implemented a hospital-community joint model for follow-up of sucking care,and the control group implemented routine discharge mode to guide nursing and teaching.The sputum withdrawal time,hospital stay, nursing satisfaction,re-admission rate,and complication score were recorded and compared between the two groups of patients with tracheotomy.Results The observation time and hospitalization time of the sputum in the observation group were shorter than that of the control group.The nursing satisfaction was higher than that of the control group.The re-admission rate and the complication score were lower than the control group within 6 months after discharge,P<0.05.Conclusion Patients with stroke tracheotomy with a hospital-community joint model of sucking management have significantly reduced the time of sucking and hospitalization,high satisfaction with nursing,and low re-admission rate and complication rate,which is worth promoting.
