槐杞黄颗粒联合激素和吗替麦考酚酯治疗儿童紫癜性肾炎的临床研究

摘　　要：目的观察槐杞黄颗粒联合激素和吗替麦考酚酯治疗儿童紫癜性肾炎(Henoch-Schonlein purpura nephritis,HSPN)的临床效果及对炎症反应、肾功能的影响。方法将2012年2月至2019年2月本院诊治的30例儿童HSPN按随机数字表法分成对照组和联合组,各15例。对照组给予醋酸泼尼松龙片和吗替麦考酚酯片治疗,联合组在对照组基础上联合槐杞黄颗粒治疗,两组均治疗3个月。观察两组临床疗效,患儿血清γ干扰素(IFN-γ)、白细胞介素6(IL-6)水平,血尿氮素(BUN)、血肌酐(SCr)水平,尿N-乙酰-β-D-葡萄糖苷酶(NAG)、β2微球蛋白(β2-MG),24h尿蛋白量、尿沉渣红细胞数量变化及不良反应发生情况。结果 联合组治疗后总有效率显著高于对照组(P<0.05);治疗后,两组血清IFN-γ、IL-6水平及BUN、SCr水平均显著降低(均P<0.05),且联合组显著低于对照组(均P<0.05);两组治疗后尿NAG、β2-MG水平及24h尿蛋白量、尿沉渣红细胞数量水平均显著降低(均P<0.05),且联合组显著低于对照组(均P<0.05);联合组不良反应发生率显著低于对照组(P< 0.05)。结论 槐杞黄颗粒联合激素和吗替麦考酚酯治疗儿童HSPN 能够更有效地降低患儿血清炎症水平和尿NAG、β2-MG 水平,降低24 h 尿蛋白量和尿沉渣红细胞数量,改善肾功能,提高安全性。Objective To observe the clinical efficacy of Huaiqihuang granule combined with hormone and mycophenolate mofetil in the treatment of Henoch-Schonlein purpura nephritis (HSPN) in children. Methods 30 children with HSPN diagnosed and treated in our hospital from February, 2012 to February, 2019 were divided into a control group and a combined group according to random number table, 15 cases for each group. The control group was treated with prednisolone acetate tablets and mycophenolate mofetil tablets, while the combined group was treated with Huaiqihuang granules on the basis of the control group;both groups were treated for 3 months. The clinical efficacy, serum levels of IFN-γ and interleukin 6 (IL-6), levels of blood urine nitrogen (BUN), serum creatinine urinary (SCr), NAG,β2-MG, and 24 h urinary protein, and urinary sediment red blood cell count and adverse reactions in the two groups were observed. Results The total effective rate of the combined group after the treatment was significantly higher than that of the control group (P < 0.05). The levels of IFN-γ, IL-6, BUN, SCr, NAG,β2-MG, and 24 h urinary protein were lower after than before the treatment in both groups (all P < 0.05), and were lower in the combined group than in the control group after the treatment (all P < 0.05). The incidence of adverse reactions was significantly lower in the combined group than in the control group (P < 0.05). Conclusion Huaiqihuang granule combined with hormone and mycophenolate mofetil tablets in the treatment of children with HSPN can more effectively reduce the levels of serum inflammation, urinary NAG, and beta 2-MG, as well as 24 h urinary protein and urinary sediment red blood cell count, and can improve the kidney function and is safe.
