摘  要:目的探究自动功能成像(AFI)在评价房颤患者(AF)左心室心肌纵向收缩功能中的价值。方法选取2018年1—12月在惠州市第一人民医院内科接受治疗的40例AF患者作为研究对象,根据左室射血分数(LVEF)水平,分为LVEF正常组和LVEF减低组,每组均为相同的20例。另选取惠州市第一人民医院内科收治的未发生过房颤且无任何心血管系统疾病的20~60岁的健康体检者20例,视为正常对照组。比较各组整体纵向应变参数(GLS-a、GLS-apex、GLS-4ch、GLS-2ch)和基底段(后间隔、前间隔、前壁、侧壁)、中段(前壁、侧壁、后壁)及心尖段侧壁等节段纵向应变参数。结果正常对照组、LVEF正常组和LVEF减低组在GLS-a、GLS-apex、GLS-4ch、GLS-2ch方面,呈现出LVEF减低组<LVEF正常组<正常对照组的规律,各组之间差异有统计学意义(P<0.05)。在基底段、中段及心尖段侧壁等阶段纵向应变参数方面,呈现出LVEF减低组<LVEF正常组<正常对照组的规律,各组之间差异有统计学意义(P<0.05)。结论AF患者早期左室心肌收缩功能正常时即可出现整体及局部应变减低,通过AFI技术能够有效且准确地对左室心肌纵向收缩功能进行评价。Objective To explore the value of AFI in evaluating the longitudinal systolic function of left ventricle in patients with atrial fibrillation(AF).Methods A total of 40 AF patients in the hospital from January to December,2018 were selected as the research objects.According to the level of LVEF,they were divided into the LVEF normal group and the LVEF reduced group,with the same number of 20 patients in each group.In addition,20 healthy subjects aged 20~60 years without atrial fibrillation and without any cardiovascular diseases admitted to the hospital were selected as the normal control group.The overall longitudinal strain parameters(GLS-a,GLS-apex,GLS-4ch,GLS-2ch),basal segment(posterior interval,anterior interval,anterior wall,lateral wall), middle segment(anterior wall,lateral wall,posterior wall)and apical segment lateral wall of each group were compared.Results In terms of GLS-a,GLS-apex,GLS-4ch and GLS-2ch,the normal control group,LVEF normal group and LVEF reduced group presented the rule of LVEF reduced group < LVEF normal group < normal control group,and the differences among the groups were statistically significant(P<0.05).In terms of the longitudinal strain parameters of the basal segment,the middle segment and the lateral wall of the apical segment,the LVEF reduced group < LVEF normal group < normal control group,and the differences among the groups were statistically significant(P<0.05).Conclusion If the left ventricular myocardial systolic function is normal in early AF patients, global and local strain reduction can be observed.AFI technique can effectively and accurately evaluate the left ventricular myocardial longitudinal systolic function.
