自拟通脉益气汤治疗扩张型心肌病心力衰竭的疗效及对炎性细胞因子、内分泌激素的影响

摘　　要：目的:观察自拟通脉益气汤治疗扩张型心肌病心力衰竭的疗效及对炎性细胞因子、神经内分泌激素的影响。方法:纳入扩张型心肌病心力衰竭病例64例,按随机数字表法分为两组,各32例。对照组采用常规西药治疗,观察组在对照组基础上增加自拟通脉益气汤治疗。比较两组疗效、治疗前后血清炎性细胞因子、神经内分泌激素水平及不良反应。结果:观察组总有效率为90.63%(29/32),明显高于对照组的68.75%(22/32),差异有统计学意义(P<0.05);治疗后,两组超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、肾素(PRA)、血管紧张素Ⅱ(AngⅡ)及醛固酮(ALD)水平均较治疗前降低,且观察组明显低于对照组,差异有统计学意义(P<0.05);观察组不良反应总发生率为21.88%(7/32),与对照组的25.00%(8/32)比较,差异无统计学意义(P>0.05)。结论:自拟通脉益气汤治疗扩张型心肌病心力衰竭疗效确切,能够缓解心衰症状,提高心功能,减轻炎症反应,调节神经内分泌因子,抑制RASS系统过度激活,抑制心室重构。Objective:To analyze effects of self-made TongmaiYiqi decoction in treatment of dilated cardiomyopathy patients with heart failure and its effects on inflammatory cytokines and neuroendocrine hormones.Methods:Sixty-four patients with dilated cardiomyopathy and heart failure were enrolled.They were divided into two groups according to the random number table method,32 cases in each.The control group was treated with conventional western medicines,while the observation group was treated with self-made TongmaiYiqi decoction.The treatment effects, the serum inflammatory cytokines and neuroendocrine hormones before and after the treatment,and the adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was 90.63%(29/32),which was significantly higher than that of the control group 68.75%(22/32),and the difference was statistically significant (P<0.05).After the treatment,the hs-CRP,IL- 6,TNF-α,PRA,AngII and ALD levels of the two groups were lower than those before the treatment,those of the observation group were significantly lower than those of the control group,and the differences were statistically significant (P<0.05).The total incidences of adverse reactions were 21.88%(7/32) in the observation group and 25.00%(8/32) in the control group,and the difference was not statistically significant (P>0.05).Conclusions:Self-made TongmaiYiqi Decoction is effective in treating dilated cardiomyopathy with heart failure.It can relieve the heart failure symptoms,improve the heart function, reduce the inflammatory reactions,regulate the neuroendocrine factors,inhibit the excessive activation of RASS system,and inhibit the ventricular remodeling.
