摘 要:目的探讨超微血流显像技术(super micro-vascular imaging,SMI)与超声造影(contrast-enhanced ultrasonography,CEUS)对颈动脉斑块内新生血管的显示能力是否具有一致性及SMI观察斑块内新生血管与脑卒中的关系。方法2016年6月至2018年3月于北京老年医院卒中科收治的符合本研究入选标准的急性脑卒中患者91例,选择同期健康体检颈动脉斑块患者75例为对照组。对患者均行颈动脉常规超声检查、SMI检查,对脑卒中患者入院24 h行CEUS检查。结果超微血管成像(SMI)与超声造影(CEUS)技术对于低回声斑块、等回声斑块、混合回声斑块新生血管的显示率差异无统计学意义(P>0.05);SMI与CEUS技术检测结果比较的一致性检验(Kappa检验),Kappa=0.735>0.60。脑卒中组SMI颈动脉斑块新生血管的检出率明显高于对照组(P<0.05)。结论SMI技术对低回声斑块、等回声斑块及混合回声斑块的新生血管均可进行观察。SMI技术可以作为一项超声诊断新技术对颈动脉斑块内的新生血管进行检测,评估斑块稳定性,斑块内新生血管越多脑卒中发生风险越大。Objective To explore the consistency of superb micro-vascular imaging(SMI)and contrast-enhanced ultrasonography(CEUS)in displaying neovascularization in carotid plaques and the relationship between plaque neovascularization and cerebral stroke.Methods A total of 91 patients with acute cerebral stroke who were admitted and treated in our hospital from June 2016 to March 2018 were enrolled in the study,at the same time,the other 75 subjects who underwent health examination for carotid plaques were enrolled as control group.All the patients underwent routine carotid ultrasonography,SMI examination,moreover,the CEUS examination was performed for the patients with cerebral stroke in 24 hours after admission.Results There was no significant difference in the display rate of neovascularization of hyperechoic plaques,isoechoic plaques and mixed echoic plaques between SMI and CEUS(P>0.05).The consistency test(Kappa test)between SMI and CEUS showed that Kappa=0.735>0.60.The detection rates of carotid plaque and neovascularization by SMI in stroke group were significantly higher than those in control group(P<0.05).Conclusion SMI technique can be used to observe the neovascularization of hyperechoic plaque,isoechoic plaque and mixed echoic plaque,and SMI and CEUS have good consistency in displaying neovascularization in carotid plaques.Moreover SMI can be used as an new ultrasound diagnostic technique to detect neovascularization in carotid plaque and evaluate the stability of plaque.The more neovascularization in plaque,the greater the risk of incidence of cerebral stroke.
