摘　　要：目的探讨COOK球囊联合缩宫素在瘢痕子宫足月妊娠阴道分娩过程中的催产效果。方法选取2016年10月~2018年4月我院产科收治的40例瘢痕子宫和40例非瘢痕子宫产妇作为研究对象,40例瘢痕子宫产妇为研究组,40例非瘢痕子宫产妇为对照组,两组均采用宫腔置入COOK球囊后12h未临产静滴缩宫素催产,比较两组产妇在采取相关措施后促宫颈成熟的总有效率、阴道分娩率、产后2h出血量、新生儿Apgar评分、临产时间、第一产程时间、总产程时间。结果研究组产妇促宫颈成熟的总有效率(77.50%)与对照组(82.50%)比较,差异无统计学意义(P>0.05);两组产妇的阴道分娩率比较,差异无统计学意义(P>0.05);两组产妇产后2h出血量、新生儿Apgar评分比较,差异无统计学意义(P>0.05);两组产妇均未发生子宫破裂;两组产妇的临产时间、第一产程时间及总产程时间比较,差异均无统计学意义(P>0.05)。结论COOK球囊联合缩宫素在瘢痕子宫足月妊娠催产中的应用和非瘢痕子宫相比同样安全有效,能够提高阴道分娩率,缩短产程,且不增加相关并发症,效果显著,值得临床推广及应用。Objective To investigate the application value of COOK balloon combined with Oxytocin in vaginal delivery during full-term pregnancy of scar uterus.Methods From October 2016 to April 2018,40 cases puerperae with scar uterus (study group) and 40 cases puerperae without scar uterus (control group) treated in Obstetrics department of our hospital were selected as the research subjects.Both groups were treated with 12-hour intrauterine placement of COOK balloon,but without delivery,after that,intravenous infusion of oxytocin was added.The total effectiveness rate of promotion of cervical maturity,vaginal delivery rate,2-hour postpartum amount of bleeding,neonatal Apgar score,time of labor,first stage of labor time,and total stage of labor time were compared between the two groups.Results There was no significant difference in the total effectiveness rate of promotion of cervical maturity between the study group (77.50%) and the control group (82.50%)(P>0.05).There was no significant difference in the vaginal delivery rate,2-hour postpartum amount of bleeding,or neonatal Apgar scores between the two groups (P>0.05).No uterine rupture occurred in both groups.The maternal labor time,first stage of labor time,and total stage of labor time were not displayed statistical significance in the two groups (P>0.05).Conclusion The application of COOK balloon combined with Oxytocin in the induction of labor in full-term pregnancy of scar uterus is safe and effective compared with non-scarred uterus.It can improve the vaginal delivery rate and shorten the labor process without increasing the related complications.The effect is remarkable and worthy of clinical promotion.
