女性产后压力性尿失禁患者膀胱颈移动度与超声尿动力学检查研究

摘　　要：目的:分析女性产后压力性尿失禁(stress urinary incontinence, SUI)患者膀胱颈移动度与超声尿动力学检查情况。方法:随机选取2015年4月至2016年4月期间在惠州市中心人民医院就诊的女性产后SUI 36例患者为试验组,无SUI患者40例为对照组;应用超声诊断仪ALOKASSD-680进行检测。结果:两组一般资料比较,差异无统计学意义(P>0.05);两组UVJ-M、ALPP、MCP、FL指标比较,差异有统计学意义(P <0.05);根据腹压漏尿点压55 cmH2O作为临界值,将SUI患者分为A组(≤55 cmH2O)和B组(>55 cmH2O),A组和B组的UVJ-M、ALPP、MCP、FL指标比较,差异有统计学意义(P <0.05),且B组中临床分度及合并盆底组织松弛情况明显优于A组,两组之间有统计学差异(P<0.05)。结论:女性产后SUI患者UVJ-M、ALPP、MCP、FL等指标显著变化,且腹压漏尿点压较低的患者,上述指标下降明显,临床分度属于II^III度,有子宫脱垂或阴道脱垂情况。Objective: To analyze the bladder neck mobility and ultrasound in patients with stress urinary incontinence in Postpartum women. Methods: Selecting randomly 36 cases for treatment of female postpartum stress urinary incontinence patients during 2015 April to 2016 April as the experimental group, no stress urinary incontinence in patients with 40 cases as control group. Results: There was no significant difference in the general data between the two groups(P >0.05).There were significant differences in the indexes of UVJ-M, ALPP, MCP and FL between the two groups(P <0.05).Patients with SUI were divided into group A(>55 cmH2 O) and group B(>55 cmH2 O) according to the critical value of abdominal leak point pressure of 55 cmH2 O.There were significant differences in UVJ-M, ALPP, MCP and FL between group A and group B(P <0.05).The clinical score and the pelvic floor tissue looseness in group B were significantly better than those in group A. Conclusion: Significant changes in UVJ-M, ALPP, MCP, FL and other indicators were observed in postpartum women with SUI. In patients with low abdominal leak point pressure, the above indicators decreased significantly. The clinical grading belongs to grade II^III, with uterine prolapse or vaginal prolapse.
