多层螺旋CT对肋骨隐匿骨折及细微骨折好发部位的诊断价值

摘　　要：目的:分析64排螺旋CT对肋骨隐匿骨折及细微骨折好发部位的诊断价值。方法:选择2015年3月至2017年12月在石家庄市第三医院行64排螺旋CT检查的100例肋骨骨折患者,首次CT检查在骨折发生1周内,并在伤后第3、4、6周进行CT复查,回顾性分析患者复查期间新检出骨折的发生情况及发生部位。结果:伤后首次CT检查共检出骨折594根,其中细微性骨折178根;复查期间有29例新检出隐匿性骨折,纳入到差异组。差异组在第3、4、6周复查时新检出隐匿性骨折分别为16、28、8根。隐匿性肋骨骨折好发于骨小梁、肋骨角及肋骨内/外缘,细微性肋骨骨折好发于肋骨角、肋骨内/外缘及肋软骨处。结论:骨小梁、肋骨内/外缘、肋骨角、肋软骨等部位易发生隐匿性及细微性骨折,临床首检后还应注意在骨折后2~6周的复查,以提高诊断准确率。Objective: To analyze the diagnostic value of 64-slice spiral CT in the occult rib fracture and slight rib fracture. Methods:100 patients with rib fractures who underwent 64-slice spiral CT examination in our hospital from March 2015 to December 2017 were selected. The first CT examination was performed within 1 week of the fracture and CT examination was performed at the3 rd, 4 th and 6 th week after injury. Retrospective analysis of the occurrence and location of new fractures during the patient review.Results: A total of 594 fractures were detected in the first CT examination after injury, including 178 minor fractures;29 new occult fractures were detected during the review and included in the difference group. In the difference group, the newly detected occult fractures were 16, 28, and 8 at the 3 rd, 4 th and 6 th week. Occult rib fractures occur in trabecular bone, rib angle and rib/outside.Slight rib fractures occur in rib angle, rib/outside and costal cartilage. Conclusion: The trabecular bone, the inner and outer ribs of the ribs, the rib angles, the costal cartilage and other parts are prone to occult and slight fractures. After the first clinical examination,attention should be paid to the re-examination of 2 to 6 weeks after the fracture to improve the diagnostic accuracy.
