五味消毒汤配合火针治疗结节囊肿型痤疮疗效及对中医症候、皮损和血清激素、炎症因子水平的影响

摘　　要：目的:观察五味消毒汤配合火针治疗结节囊肿型痤疮疗效及对中医症候、皮损和血清激素、炎症因子水平的影响。方法:选取2017年6月~2018年6月我院收治的寻常型痤疮100例,采用随机数字表法分为治疗组与对照组各50例,对照组予以夫西地酸软膏常规治疗,治疗组采用五味消毒汤配合火针治疗,两组患者均以21天为1个疗程,观察两组患者治疗有效率及不良反应发生情况,治疗前后中医证候积分、皮损积分、血清激素及炎症因子变化。结果:两组患者治疗有效率分别为88.0%、68.0%,治疗组明显高于对照组(P<0.05);两组患者治疗后中医证候积分及皮损积分较治疗前均明显降低(P<0.05),且治疗组显著低于对照组(P<0.05);治疗后血清游离睾酮(FT)、雌二醇(E2)、肿瘤坏死因子-α(TNF-α)、白介素-8(IL-8)水平较治疗前均明显下降(P<0.05),且治疗组明显低于对照组(P<0.05);患者均无明显不良反应。结论:五味消毒汤配合火针内外结合治疗可有效提高结节囊肿型痤疮治疗效果,显著改善皮损症状,抑制炎症反应,调节机体内分泌功能,疗效确切,安全有效。Objective: To analyze the curative effect of using Wuwei Xiaodu Decoction combined with fire needle in the treatment of nodular-cystic acne and its influence on TCM symptoms,skin lesions,serum hormones and inflammatory factors. Methods: 100patients with acne vulgaris who were admitted to the hospital from June 2017to June 2018were randomly divided into treatment group and controlled group,50patients in each group. The controlled group was treated with fusidic acid ointment,and the treatment group was treated with Wuwei Xiaodu Decoction combined with fire needle. 21days was a course of treatment. The treatment response rates,incidence of adverse reactions,changes of TCM syndrome scores,lesion scores,serum hormones and inflammatory factors in both groups before and after treatment were observed. Results: The treatment response rate of the treatment group was significantly higher than the controlled group ( 88. 0% vs 68. 0%)( P<0. 05). The TCM syndrome scores and lesion scores of both groups were significantly decreased after treatment ( P<0. 05),showing the treatment grouP<the controlled group ( P<0. 05). Levels of serum free testosterone ( FT),estradiol ( E2),tumor necrosis factor-α( TNF-α) and interleukin- 8 ( IL-8) were significantly decreased after treatment ( P < 0. 05),showing the treatment grouP < the controlled group ( P < 0. 05). There were no obvious adverse reactions. Conclusion: Wuwei Xiaodu Decoction combined with fire needle can effectively improve the curative effect on nodular-cystic acne,significantly relieve skin lesions,inhibit inflammatory response,and regulate the secretion function,safe and effective.
