茯苓导水汤联合激素加环磷酰胺治疗膜性肾病临床观察

摘　　要：目的探寻茯苓导水汤联合激素加环磷酰胺治疗膜性肾病临床疗效。方法选取2017年9月—2019年2月辽宁中医药大学附属医院肾内科经肾活检或PLA2R检测证实的成人膜性肾病患者38例,对照组患者采用激素联合环磷酰胺治疗,治疗组患者采用茯苓导水汤联合激素加环磷酰胺治疗,治疗6个月后,比较患者的临床症状改善情况、24 h尿蛋白定量、血浆白蛋白及有效率。结果化验指标及中医证候积分在治疗前2组差异无统计学意义(P>0.05),经过分组治疗后治疗组患者化验指标及中医证候的改善情况更加明显(P<0.05)。结论茯苓导水汤联合激素加环磷酰胺治疗膜性肾病效果较好。Objective:To study the clinical effect of Poria Cocos Decoction combined with hormone plus cyclophosphamide on the treatment of membranous nephropathy. Methods: 38 patients with adult membranous nephropathy who were confirmed by renal biopsy or PLA2 R in the Department of Nephrology in The Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from September 2017 to February 1919 were selected and divided into a western medicine group and a Chinese and Western medicine group. The western medicine group was treated with hormone combined with cyclophosphamide and the Chinese and western medicine group were treated with Poria Cocos Decoction combined hormone with cyclophosphamide. After 6-month treatment, the clinical symptoms improvement, 24-hour urine protein quantitation, plasma albumin and effective rate of the patients were compared. Results: The above test indicators and TCM symptom scores were not significantly different between the two groups before treatment(P>0.05). After grouping treatment, the improvement of the test indicators and TCM symptoms in the Chinese and western medicine group was more obvious and the curative effective rate was more higher than that of the western medicine group. The high rate was 80.95% and the symptomatic treatment effect rate was 85.71%, higher than that of the western medicine group. The difference between the groups was statistically significant(P<0.05). Conclusion: Poria Cocos Decoction combined with hormone plus cyclophosphamide has a significant effect on the treatment of membranous nephropathy and has greater advantages and broader development prospects and is worthy of clinical application and promotion.
