达比加群酯治疗胸腹腔镜术后下肢静脉血栓的临床疗效

摘　　要：目的探讨达比加群酯治疗胸腹腔镜术后下肢静脉血栓的临床疗效。方法选取武汉科技大学附属天佑医院肝胆普胸外科2016年6月-2018年1月收治的胸腹腔镜术后下肢静脉血栓患者92例,随机分为对照组和观察组,各46例。对照组患者给予华法林片治疗,观察组患者给予达比加群酯胶囊治疗。两组患者均维持治疗3个月,随访3个月。比较两组患者临床疗效,治疗前及治疗后1周、1个月、3个月、6个月凝血指标〔血小板(PLT)、部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、D-二聚体和国际标准化比值(INR)〕,并观察两组患者不良反应发生情况。结果观察组患者总有效率高于对照组(P<0.05)。治疗前两组患者PLT、APTT、FIB、D-二聚体、INR比较,差异无统计学意义(P<0.05);治疗后1周、1个月、3个月、6个月观察组患者PLT、FIB、D-二聚体低于对照组,APTT长于对照组,INR高于对照组(P<0.05)。观察组患者不良反应发生率低于对照组(P<0.05)。结论达比加群酯治疗胸腹腔镜术后下肢静脉血栓的临床疗效较好,可有效改善患者凝血指标,提高抗凝作用,且安全性较高。Objective To investigate the preventive effect of dapigatron ester on lower extremity venous thrombosis after thoracoscopic and laparoscopic surgery.Methods A total of 92 cases of patients with lower extremity venous thrombosis after thoracoscopic and laparoscopic surgery were selected from June 2016 to January 2018 in Department of Hepatobiliary and Thoracic Surgery,Tianyou Hospital Affiliated to Wuhan University of Science & Technology,which were randomly divided into control group and observation group,46 cases in each group.The control group were given warfarin tablets,while the observation group were given dapigatron ester capsules.Both groups were treated for 3 months,followed up for 3 months.The clinical efficacy,the coagulation indexes including PLT,APTT,FIB,D-dimer and INR were observed before and after treatment of 1 week,1 month,3 months and 6 months.The occurrence of adverse reactions during treatment was observed.Results The total effective rate of the observation group was higher than control group(P<0.05).There was no significant differences in PLT, APTT,FIB,D-dimer or INR between the two groups before treatment(P<0.05);after treatment of 1 week,1 month, 3 months and 6 months,the observation group of the PLT,FIB and D-dimer were lower than control group,APTT was longer than control group,and INR was higher than control group(P<0.05).The incidence rate of adverse reactions in the observation group was lower than control group(P<0.05).Conclusion Dapigatron has an effective in preventing lower extremity venous thrombosis after thoracoscopic and laparoscopic surgery,it can effectively improve coagulation index and anticoagulation effect, and with high safety.
