摘  要:目的总结椎间孔镜下可视化关节突成型技术治疗腰椎间盘突出症(LDH)的短期疗效及经验。方法回顾性分析可视化关节突成型技术治疗28例LDH的临床资料,采用视觉模拟评分法(VAS)、Oswestry功能障碍指数(ODI)及日本骨科协会(JOA)评分来评价手术前后腰腿痛程度变化及功能恢复程度,运用改良MacNab疗效评分标准记录末次随访疗效。结果28例病人均顺利完成手术。术后各时间点VAS、ODI及JOA评分与术前比较,差异均有统计学意义(P <0.01)。根据术后6个月改良MacNab疗效评分标准:优18例,良6例,可3例,差1例,总体优良率为85.7%。结论可视化关节突成型技术具有创伤小、麻醉风险小、出血少、恢复快、费用低等优点,是治疗LDH安全、有效的方法;但远期效果仍然有待有一步观察。Objective To summarize the short-term efficacy and experience of visual molding technique of articular process under an intervertebral foramen endoscope for the treatment of lumbar disc herniation(LDH). Methods Clinical data of 28 LDH patients undergoing visual molding technique of articular process were analyzed retrospectively. The changes of lumbocrural pain intensity and functional recovery were evaluated by visual analogue scale(VAS), Oswestry disability index(ODI) and Japanese orthopaedic association(JOA) before and after the surgery, and the efficacy of the last follow-up was recorded by modified MacNab efficacy score.Results All 28 patients were accomplished the surgery. There were significant differences in VAS, ODI and JOA scores between before and different time points after the surgery(P < 0.01). According to the modified MacNab efficacy score 6 months after surgery:excellent result was achieved in 18 patients, good in 6, moderate in 3 and bad in 1, and the rate of excellent and good outcome was85.7%. Conclusions Visual molding technique of articular process has the advantages as less trauma, less risk of anesthesia, less bleeding, rapid recovery and low cost. It is a safe and effective method for treating LDH, but the long-term effect remains to be observed further.
