辅助生殖技术双胎妊娠子代神经发育的随访研究

摘　　要：目的探讨辅助生殖技术(ART)双胎妊娠对子代神经发育的主要影响因素。方法选取2011年1月-2017年12月在深圳市妇幼保健院儿童保健科就诊双胎妊娠出生的0~3岁婴幼儿2538例为ART双胎组,1728例为自然双胎组。结果两组间分娩时孕周和出生体质量以及月龄、身长/高、体质量、MDI和MDI2差异均无统计学意义(P>0.05);ART双胎组的Apgar1和Apgar5、剖宫产比例和人工喂养比例均高于自然双胎组,差异有统计学意义(P<0.05);ART双胎组的PDI和PDI2高于自然双胎组,差异有统计学意义(P<0.05);两组MDI和PDI与孕周、出生体质量均有一定的相关性,且相关系数均比较相近(接近0.45);而MDI2和PDI2与孕周、出生体质量的相关性不强(接近0.1)。结论双胎妊娠的子代神经发育与妊娠方式无关,家长的良好教育理念、好的家庭氛围、丰富的信息刺激,以及延长胎龄将对婴幼儿神经发育有积极的作用。Objective To investigate the main influencing factors of the twin pregnancy of assisted reproductive technology( ART) about neurogenesis development. Methods Between January 2011 and December 2017,2 538 cases of infants aged 0 ~ 3 years with twin pregnancies in the children ’ s health department of our hospital were selected as ART twin groups, and 1 728 were natural twin groups. Results There was no significant difference between the gestational weeks and the birth weight of the two groups,as well as the monthly age,length/height,weight,MDI and MDI2 there was not significant differences between the two groups( all P>0. 05);The Apgar1 and Apgar5 scores,cesarean section ratio and artificial feeding ratio of ART twin groups were higher than that of natural twin groups,the difference was statistically significant( P< 0. 05). The PDI and PDI2 were higher than those in the natural twins,the difference was statistically significant( P<0. 05). Both groups of MDI and PDI were correlated with gestational age and birth weight,and the correlation coefficients were similar( Close to 0. 45);The correlation between MDI2 and PDI2 after correction was not strong with gestational weeks and birth weight( close to 0. 1). Conclusion Offspring of twin pregnancy,neurogenesis development has nothing to do with the way of pregnancy,may parents’ good education idea,good family atmosphere,abundant information,and will extend gestational age infant psychomotor development have a positive effect.
