西藏高原地区藏族高血压患者血浆致动脉硬化指数与左心室舒张功能相关性分析

摘　　要：目的探讨西藏高原地区藏族原发性高血压患者血浆致动脉硬化指数(atherogenic index of plasma,AIP)与左心室舒张功能的相关性.方法选择我科初次诊断为原发性高血压患者107例,按是否存在左心室舒张功能不全分为两组,并选择血压正常的患者为对照组(45例).所有患者入院后在服药前均检测血生化各指标水平,并计算血浆致动脉硬化指数(AIP=IgTG/HDL),采用心脏多普勒超声评价左心室舒张功,并分析AIP与左心室舒张功能的相关性.结果患者AIP与E/A呈负相关(r=-0.690,P<0.01),AIP与E/Ea呈正相关(r=0.334,P<0.023).高血压左心室舒张功能不全患者AIP的ROC曲线分析提示曲线下面积=0.951.Logistic回归分析提示AIP是左心室舒张功能减退独立相关因素(P<0.01).结论 AIP可能是西藏高原地区藏族原发性高血压患者早期出现左心室舒张功能不全的一个标志.Objective To investigate the relationship between atherogenic index of plasma and left ventricular diastolic function in Tibetan patients with essential hypertension in Tibet plateau area. Methods 107 Tibetan patients who had being diagnosed as essential hypertension were divided into two groups, according to being with or without left ventricular diastolic dysfunction group. At the same time 45 Tibetan patients with normal blood pressure were enrolled into control group. Serum biochemical index were measured before treatment and the atherogenic index of plasma was calculated (AIP=lgTG/HDL). Left ventricular diastolic function was assessed by pulsed Doppler tissue imaging. Subsequently, compared the relationship between AIP and left ventricular diastolic function was analized. Results The AIP was negatively correlated with E/A(r=-0.690, P<0.01) and positively correlated with E/Ea (r=0.334, Pv0.023). The Roc curve analysis of AIP in hypertensive patients with left ventricular diastolic dysfunction showed: AUC=0.951. Logistic regression analysis showed that AIP was an independent factor associated with left ventricular diastolic dysfunction (P<0.01). Conclusion AIP might be a marker of diastolic dysfunction early in essential hypertension patients in Tibet plateau area.
