岛叶与侧脑室的显微解剖关系及临床意义

摘　　要：目的探讨岛叶与侧脑室的解剖对应关系,为该部位病变的手术治疗提供显微形态学基础及术中标志性解剖学参数。方法收集国人成人完整头颅湿标本10例(20侧),显微镜下解剖岛叶和侧脑室区,观察岛叶与侧脑室的解剖对应关系,测量相关数据并拍照。结果岛叶前界沟深部与侧脑室额角相对应;上界沟由前至后分别与侧脑室额角、体部、三角部相对应,并由岛叶深部的基底核、内囊所分隔;下界沟分别与后方的侧脑室三角部和下方的颞角相对应。前岛点至侧脑室额角外侧壁的垂直距离为(11.56±2.52) mm,后岛点至侧脑室三角部外侧壁的垂直距离为(10.94±2.65) mm。下界沟下缘终点至侧脑室颞角外侧壁的垂直距离为(9.36±1.67) mm。结论岛叶与侧脑室各部间存在相对固定的对应关系,熟练掌握解剖对应关系,有利于岛叶病变准确切除,同时尽量减少手术的副损伤。Objective To explore the anatomically corresponding relationship between the insula and lateral ventricles for providing the micromorphological base and symbolic anatomic parameters during the surgery. Methods Ten wet specimen(20 sides) of intact adult Chinese cadaveric heads were collected. The insula and lateral ventricles were dissected under a microscope to observe the anatomically corresponding relationship. The relevant data were measured and photographs were taken. Results The deep part of the anterior sulcus of the insula corresponds to the frontal angle of the lateral ventricle. The superior sulcus corresponds to the frontal angle, body and triangle of the lateral ventricle respectively from the anterior to posterior, which is separated by the basal nucleus and internal capsule of the deep part of insula. The superior sulcus corresponds to the triangle of the posterior lateral ventricle and the temporal angle in the lower part, respectively. The vertical distance between the anterior insular point and the lateral wall of frontal horn of lateral ventricle is 11.56 ± 2.52 mm. The vertical distance between the posterior insular point and the lateral wall of the triangle of the lateral ventricle was 10.94 ± 2.65 mm. The terminal point of inferior margin of inferior sulcus and the lateral wall of frontal angle was 9.36 ± 1.67 mm. Conclusions There are relatively fixed corresponding relationship between the insula and every parts of the lateral ventricle. The anatomically corresponding relationship should be mastered, which is helpful in resecting the insular lesions precisely and reducing the surgical injury as much as possible.
