摘  要:目的检测DKD患者肾脏组织中白介素10(IL-10)及DNA甲基转移酶1(Dnmt-1)的表达,探讨两者关系并分析其与各临床指标的关系。方法选取新乡医学院第一附属医院肾内科住院并经肾活检确诊为DKD患者23例为实验组(DKD),9例病理诊断为轻微病变型肾小球疾病为对照组(Con),免疫组织化学法检测IL-10、Dnmt-1的表达,比较其表达有无明显差异,并分析两指标与其他临床资料之间的关系。结果DKD组较Con组患者尿蛋白、胱抑素C升高,血沉加快,血清白蛋白、红细胞计数降低等特点。两组IL-10表达比较差异无统计学意义(z=-0.315,P=0.752)。DKD组Dnmt-1表达高于Con组,差异有统计学意义[43.0(31.0,48.0)vs 0.30(0.05,0.35),z=-4.434,P=0.000]。DKD组IL-10与Dnmt-1呈负相关(r=-0.434,P=0.038)。结论DKD患者肾组织中可检测到抗炎因子IL-10的表达,且IL-10的分泌与Dnmt-1表达呈负相关。Objective To investigate the expression of interleukin IL-10 and DNA methylation transferase-1(Dnmt-1)in renal tissue in patients with type 2 diabetic nephropathy(DKD),then investigate the relationship between the two indicators and analyze their relationship with the clinical indicators.Methods A total of 23 patients with DKD diagnosed by renal biopsy were chosen as experimental group(DKD group),and 9 patients with mild lesion glomerular disease diagnosed by renal biopsy were chosen as control group(Con group). The expression of IL-10 and Dnmt-1 were tested by immunohistochemistry. The difference of the expression was compared and the relationship between the two indicators and the clinical data were analyzed.Results Compared with the Con group,DKD group showed significant obvious characteristics of proteinuria,higher cystatin C,faster erythrocyte sedimentation,lower serum albumin,and red blood cell counts. IL-10 was expressed mainly in renal tubular cytoplasm,and the Mann-Whiney U of Nonparametric test showed that there was no significant difference between DKD group and Con group(z=-0. 315,P=0. 752). The expression of Dnmt-1 was mainly in nucleus of renal tubular cells in DKD group,and the expression level of Dnmt-1 was significantly higher in DKD group than in Con group[43. 0(31. 0,48. 0)vs0. 30(0. 05,0. 35),z=-4. 434,P=0. 000]. The expression of IL-10 was negatively correlated with Dnmt-1 in DKD group(r=-0. 434,P=0. 038).Conclusion The expression of the anti-inflammatory factor IL-10 could be detected in the renal tissue of DKD,and the secretion of IL-10 was negatively correlated with the expression of Dnmt-1.
