摘 要:目的:观察针刀干预对颈椎病兔椎间盘软骨细胞凋亡的影响,探讨针刀治疗颈椎病可能的疗效机制。方法:将40只新西兰兔随机分为空白组、模型组、电针组及针刀组,使用自制低头位造模装置建立颈椎病兔模型,行针刀及电针干预,针刀组选取C2~C7棘突旁开1~1.5 cm处和C2~C7棘突点的条索状物进行松解、剥离,电针组选取双侧天柱、百劳、大杼穴,进行电针干预,干预结束后观察颈椎间盘形态学改变,并采用TUNEL法检测各组动物颈椎间盘阳性细胞核数。结果:与空白组比较,模型组TUNEL阳性细胞核数目显著增加(P<0.05);与模型组比较,针刀组阳性细胞核数明显减少(P<0.05),电针组阳性细胞核数减少(P<0.05)。结论:低头位法可增加颈椎病模型兔软骨终板凋亡细胞数目,针刀干预可显著减少颈椎间盘软骨细胞凋亡数目,这可能是针刀治疗颈椎病的疗效机制之一。Objective:To observe the effects of acupotomytherapy on the nucleus count of TUNEL-positive nuclei in the cervical endplate of rabbits with cervical spondylosisand explore the possible mechanism of acupotomy for treating cervical spondylosis. Methods:Forty New Zealand rabbits were randomly divided into control group, model group, electroacupuncture group and needle-knife group. The cervical spondylosis model was established by using self-made rabbits with lower head.The acupotomy group and the electro-acupuncture group were selected from C2-C7 spinous process 1 to 1.5 cm apart and C2-C7 spinous process points were used for acupotomy and electro-acupuncture. After intervention, the morphological changes of cervical intervertebral disc were observed. The TUNEL method was used to detect the number of positive nuclei in cartilage endplates of experimental rabbits. Results:Compared with the control group, the number of TUNEL-positive nuclei in the model group increased significantly(P<0.05);the number of TUNEL-positive nuclei in the acupotomy group decreased significantly(P<0.05);compared with the model group,the number of TUNEL-positive nuclei inthe electro-acupuncture group was decreased(P<0.05). Conclusion: The low head position method can increase the number of apoptotic cells in the endplate of cartilage of cervicalspondylosis model. Acupotomytherapy can significantly reduce the number of apoptotic cells, which may be one of the mechanisms ofneedle-knife treating cervical spondylosis.
