摘  要:目的:探讨广藿香酮对分泌性中耳炎模型豚鼠耳黏膜中肿瘤坏死因子-α(TNF-α)及细胞间黏附分子(ICAM-1)表达的影响。方法:选取健康豚鼠82只,雌雄各半,随机分为对照组16只和造模组66只。造模组豚鼠通过灭活细菌诱导法建立分泌性中耳炎模型,对照组则给予等量生理盐水注射。将造模成功豚鼠随机分为模型组、广藿香酮低、高剂量组以及泼尼松组,各16只。模型组和对照组分别灌胃给予对照溶液2.5 mL,1次/d;广藿香酮低、高剂量组分别灌胃给予1 mg/mL、4 mg/mL广藿香酮溶液2.5 mL,1次/d;泼尼松组灌胃给予0.8 g/mL泼尼松溶液2.5 mL,1次/d。各组豚鼠均连续给药7 d。检测听性脑干反应(ABR)指标、中耳黏膜厚度及中性粒细胞计数、耳泡灌洗液TNF-α、可溶性细胞间黏附分子(sICAM-1)水平、耳黏膜TNF-α、ICAM-1表达水平。结果:治疗后,与模型组比较,各组豚鼠刺激强度90 dB时ABRⅠ、Ⅲ波潜伏期较短,ABR反应阈较低,各组豚鼠中耳黏膜厚度及中性粒细胞计数较低,各组豚鼠耳泡灌洗液TNF-α、sICAM-1水平较低,各组豚鼠中耳黏膜TNF-α、ICAM-1灰度值较低,且广藿香酮低剂量组>广藿香酮高剂量组、泼尼松组,差异有统计学意义(P<0.05)。结论:广藿香酮能明显减轻分泌性中耳炎模型豚鼠听力损失,减轻黏膜厚度和中性粒细胞浸润,其作用可能与抑制耳黏膜中TNF-α及ICAM-1表达有关。Objective: To analyze the effect of patchouli ketone on the expressions of tumor necrosis factor-α(TNF-α) and intercellular adhesion molecule(ICAM-1) in the ear mucous membrane of guinea pigs with secretory otitis media. Methods: Eighty-two healthy guinea pigs, sex in half, were randomly divided into control group with 16 guinea pigs and model building group with 66 guinea pigs. The model of secretory otitis media was established by inactivated bacterial induction. The control group was given equal amount of saline injection. The successfully modeled guinea pigs were randomly divided into model group, patchouli ketone low-dose, high-dose groups and prednisone group, each with 16 guinea pigs. The model group and the control group were respectively given the control solution 2.5 mL by gavage, once a day. The patchouli ketone low-dose, high-dose groups were respectively given the 1 mg/mL, 4 mg/mL patchouli ketone solution 2.5 mL by gavage, once a day. The prednisone group was given 0.8 g/mL prednisone solution 2.5 mL by gavage, once a day. Each group was given a continuous dose for 7 days. The indexes of auditory brainstem response(ABR), the thickness of the middle ear mucosa and the count of neutrophils, the levels of TNF-α and soluble intercellular adhesion molecule(sICAM-1) in the alveolar lavage fluid,expression levels of TNF-α and ICAM-1 in the ear mucosa were detected. Results: After treatment, compared with model group, the latencies of ABR I and III wave at 90 dB of stimulation intensity were shorter, ABR reaction threshold was lower in each group and the thickness of the middle ear mucosa and the count of neutrophils in the middle ear of the guinea pigs were lower in each group. The levels of TNF-α and sICAM-1 in the alveolar lavage liquid of guinea pig in each group were lower. The gray values of TNF-α and ICAM-1 in the middle ear mucosa of the guinea pigs were lower in each group and that of the patchouli ketone low-dose was more than that of high-dose group and prednisone group.
