注射用血栓通(冻干)治疗视网膜中央静脉阻塞的临床疗效研究

摘　　要：目的初步评价注射用血栓通(冻干)治疗证属血瘀的非缺血型视网膜中央静脉阻塞的有效性,为后续研究提供依据。方法收集在2015年6月10日-2017年11月2日就诊的"血瘀型"非缺血型视网膜中央静脉阻塞患者,共计130例(130只眼),治疗组65例,对照组65例,疗程10~14 d。比较治疗前后的最佳矫正视力(BCVA)、视功能损害眼病患者生存质量(SQL-VI)评分、视野、中医证候积分。结果治疗后2组患者视力、视野情况明显改善,治疗组的治疗效果优于对照组,log MAR视力:t=-2.92,P=0.008;视野:χ^2=8.21,P=0.042,均有统计学意义。与治疗前相比,2组患者的视功能损害眼病患者生存质量(SQL-VI)(t=-2.53,P=0.013)和生存质量(自评)评分(t=4.19,P<0.001)均有明显的提升,差异有统计学意义。中医证候总积分的组间比较,治疗组优于对照组,差异有统计学意义(t=-3.16,P<0.05)。治疗组的胸胁胀痛、精神抑郁及烦躁失眠的改善情况优于对照组,差异有统计学意义(P<0.05)。结论注射用血栓通(冻干)能够有效治疗视网膜中央静脉阻塞,能提高患者的视力和生活质量,改善视野、中医证候及症状体征等情况。OBJECTIVE To evaluate the effectiveness of the non-ischemic central retinal vein occlusion in blood stasis syndrome by Xueshuantong flyophilized powder for injection, and to provide basis for the follow-up studies. METHODS One hundred and thirty non-ischemic central retinal vein occlusion patients(130 eyes) in blood stasis type were multicenter collected from June10 th, 2015 to November 2 nd, 2017, of which 65 cases were in treatment group and 65 cases in control group. The course lasted for 10-14 days. The best corrected visual acuity(BCVA), the quality of life(SQL-VI) score, visual field, and TCM syndrome scores of patients with visual impairment before and after treatment were compared. RESULTS After treatment, the visual acuity of the two groups of subjects both improved significantly. The therapeutic effect of the treatment group was better than that of the control group. For that the log MAR of vision t =-2.92,P =0.008;The visual field, χ2=8.21,P=0.042, the differences were statistically significant. The SQL-VI(t=-2.53,P=0.013)and quality of life(self-evaluation) score(t=4.19,P<0.001)were significantly increased in both groups of patients with visual impairment, the difference was statistically significant;The total scores of TCM syndromes in the treatment group were better than those in the control group, and the difference was statistically significant(t =-3.16,P =0.002).The effect on symptoms as chest and hypochondrium distending pain, mental depression and insomnia due to irritability was better in treatment group than that in control group(P<0.05). CONCLUSIONS The Xueshuantong lyophilized powder for injection was effective on patients with central retinal vein occlusion, in that it obviously enhanced vision and quality of life, and improved their visual field, situation of TCM syndromes,signs and symptoms.
