摘　　要：目的比较Graves病(Graves’disease,GD)患者血小板参数与血清促甲状腺激素受体抗体(thyrotropin receptor antibodies,TRAb)水平之间的相关性。方法选取GD患者81例(GD组)及健康体检者27例(对照组),根据甲功状态将GD患者分为初发组(25例)、部分缓解组(18例)、完全缓解组(38例)三个亚组。所有受试者测定血清TRAb、血小板参数、IgG型抗血小板抗体(PA-IgG)。结果与对照组相比,平均血小板容积(MPV)、血小板体积分布宽度(PDW)和大血小板比率(P-LCR)在GD组显著升高(P<0.01),且与血清TRAb呈正相关(P<0.01)。其中,GD组患者P-LCR与血清TRAb之间存在着更密切的相关性。血小板计数(PLT)及PA-IgG阳性率在GD组与对照组间无统计学差异(P>0.05)。结论血小板体积参数有望成为GD的早期诊断、药物疗效和疾病转归判定的新候选指标。Objective To compare the correlation between platelet parameters and serum thyrotropin receptor antibodies(TRAb) in patients with Graves’ disease(GD). Methods Eighty-one patients with GD(GD group) and 27 healthy persons(control group) were selected. According to the status of thyroid function, GD patients were divided into three subgroups: incipient GD group(25 cases), partial remission group(18 cases) and complete remission group(38 cases). Serum TRAb, platelet parameters and IgG antiplatelet antibody(PA-IgG) were measured in all subjects. Results Compared with the control group, mean platelet volume(MPV), platelet distribution width(PDW) and platelet-large cell ratio(P-LCR) were significantly increased in GD group(P<0.01), and were positively correlated with serum TRAb(P<0.01). Among them, there was a closer correlation between P-LCR and serum TRAb in GD patients. There was no significant difference in platelet counts(PLT) and PA-IgG positive rate between GD group and control group(P>0.05). Conclusion Platelet volume parameters have reason to be a new candidate index for early diagnosis, drug efficacy and prognosis of GD.
