活血化瘀方蜡疗联合辽妥对血液透析患者自体动静脉内瘘功能的影响

摘　　要：目的:观察活血化瘀方蜡疗联合喜疗妥干预对血液透析患者自体动静脉内瘘功能的影响。方法:选择96例使用自体动静脉内瘘行维持性血液透析的患者为研究对象,随机分为观察组和对照组。对照组48例常规使用喜辽妥软膏于动静脉内瘘穿刺24小时后外涂,观察组48例采用活血化瘀方蜡疗联合喜辽妥软膏外涂。比较两组患者治疗前后在内瘘穿刺成功率、内瘘血流量等方面的情况,比较两组患者12个月内瘘皮肤瘢痕形成及发生内瘘血管并发症等方面的情况。结果:两组患者动静脉内瘘穿刺成功率、内瘘血流量均比治疗前提高,且观察组疗效优于对照组,差异有统计学意义(P<0.05)、观察组患者内瘘皮肤疤痕形成及范围、动静脉内瘘血管并发症发生率均低于对照组,差异有统计学意义(P<0.05)。结论:活血化瘀方蜡疗联合喜辽妥早期干预能提高AVF穿刺成功率,改善AVF血流量,有效预防皮肤瘢痕形成,降低血管并发症的发生率,具有较高的临床应用价值,可推广使用。Objective: To explore the effect of Huoxue Huayu Recipe wax therapy combined with Hirudoid Cream on arteriovenous fistula ( AVF) function in hemodialysis patients. Methods: 96maintenance hemodialysis patients with AVF were randomly divided into observation group and controlled group,in the controlled group,48cases were routinely applied with Hirudoid Cream 24hours after the puncture of AVF,and 48cases in the observation group were treated with Huoxue Huayu Recipe wax therapy combined with Hirudoid Cream. Compared the success rate of AVF puncture,AVF blood flow,the AVF skin scar formation and complications before and after treatment between the two groups. Results: The success rate of puncture and the blood flow of AVF in the two groups were both higher than before treatment,and the efficacy of the observation group was better than that of the controlled group,with statistically significant difference ( P<0. 05),the scar formation and scope of AVF skin and the incidence of AVF complications in the observation group were lower than those in the controlled group,with statistically significant difference ( P<0. 05). Conclusion: Early intervention of Huoxue Huayu Recipe wax therapy combined with Hirudoid Cream can increase the success rate of AVF puncture,improve the AVF blood flow,effectively prevent the formation of skin scar and reduce the incidence of AVF complications,which has high clinical application value and can be widely used.
