摘 要:目的研究2016年1月至12月广西医科大学第一附属医院临床分离的鲍曼不动杆菌的临床分布及耐药性情况,为临床合理使用抗菌药物提供参考。方法回顾性分析2016年在广西医科大学第一附属医院就诊的门急诊患者和住院患者中分离的鲍曼不动杆菌,对其细菌分布及其药物敏感性进行分析。结果共分离出鲍曼不动杆菌1165株,99.66%菌株分离自住院患者,65.84%菌株分离自痰标本。鲍曼不动杆菌对抗菌药物呈高度耐药。亚胺培南和美罗培南的耐药率分别为85.60%和83.08%。头孢哌酮/舒巴坦和哌拉西林/三唑巴坦的耐药率分别为83.00%和82.40%。ICU科分离的鲍曼不动杆菌耐药率最高,其次是儿科和内科,外科分离的鲍曼不动杆菌耐药率最低。痰标本中分离的鲍曼不动杆菌对抗菌药物的耐药率最高,血液标本分离的鲍曼不动杆菌对抗菌药物的耐药率最低。结论我院分离的鲍曼不动杆菌耐药情况严重;分离自不同年龄段、科室和标本类型的菌株对抗菌药物的耐药性相差较大。医院应继续开展病原菌分布及其耐药性监测,用于指导临床医生合理使用抗生素,控制耐药菌株在医院中的流行。Objective To investigate the distribution and antibiotic resistance of Acinetobacter baumannii in the first affiliated hospital of Guangxi medical university in 2016, so as to provide reference for rational use of antibiotics. Methods Retrospective analysis of the distribution and antibiotic resistance of Acinetobacter baumannii in 2016 in the first affiliated hospital of Guangxi medical university. Results A total of 1165 isolates of Acinetobacter baumannii were collected in the first affiliated hospital of Guangxi medical university. Majority of 1165 isolates of Acinetobacter baumannii were from inpatients ( 99.66%) and sputum samples ( 65.84%). Acinetobacter baumannii was highly resistant to antibiotics. The resistance rates of Acinetobacter baumannii to imipenem and meropenem were 85.60% and 83.08%, respectively. The resistance rates of Acinetobacter baumannii to cefoperazone / sulbactam and piperacillin / tazobactam were 83.00% and 82.40%, respectively. The resistance rate of Acinetobacter baumannii isolated from ICU were the highest, followed by pediatrics and internal medicine, the lowest resistance rates of Acinetobacter baumannii were from surgery medicine. The resistance rates of Acinetobacter baumannii isolated from sputum sample were the highest, and the lowest resistance rates of Acinetobacter baumannii were from blood sample. Conclusions The resistance rates of Acinetobacter baumannii collected from the first affiliated hospital of Guangxi medical university are serious. The resistance rates of Acinetobacter baumannii vary with patient age, specimen type and the department where the strain was isolated. Surveillance of antimicrobial resistance is important for rational antimicrobial therapy and effective prevention and control of the spread of resistant Acinetobacter baumannii strains.
